Form of workThis is a temporary, grant funded position that is expected to last for one year with the possibility of being extended if more funding is secured. Nursing care in the Internal Medicine Clinic is part of a multi-disciplinary health care team. Care is provided to patients of varying socioeconomic & cultural backgrounds in an outpatient setting, primarily within the Ryan White HIV/AIDS Program. This position will be working with our PREP home study as well.
BSN rates $51.50-$78.36 per hour, based on education and experience. ADN and MSN rates also available.
- Intake of New Patients:
- Responsible for the initial assessment and intake of new HIV patients to the clinic.
- Point of Contact for Patients:
- Acts as a primary contact for patients, addressing their medical issues and concerns.
- Guidance on Antiretrovirals and Adherence:
- Provides information about antiretroviral therapy and the importance of adherence to these treatments.
- Diagnosis and Treatment of STIs:
- Diagnoses and treats STIs per established protocols.
- Coordinates STI treatment with community health providers.
- Monitoring and Reporting:
- Regularly monitors patients' health status, including viral load and CD4 counts, and reporting any significant changes to providers
- Care Coordination Across Services:
- Ensuring a holistic approach to patient care by coordinating with case management including Partnership Project and Russel Street
- Sexual Health Counseling and Adherence Support:
- Provides counseling on safe sex practices and adherence to treatment plans.
- Hepatitis C Care Coordination:
- Assists in the coordination of Hepatitis C treatment and follow-up care for co-infected patients.
- Research and Continuous Learning:
- Keeps up-to-date with the latest in HIV treatment and care, by attending HIV related conferences- such as ANAC. This will be supported if there are funds.
- Crisis Intervention:
- Provides support and intervention in crisis situations, related to health emergencies, mental health issues, or social crises.
- Participation in Quality and Team Meetings:
- Attends meetings focused on HIV care quality, including discussions on patients lost to care.
- Documentation and Time Management:
- Logs the time used for providing care to patients who are eligible for Ryan White services.
- External applicants- BSN required
- Communication skills including fluency in oral and written English
- Basic computer skills including the ability to send/receive email, navigate information technology associated with the position, and use Electronic Health Record information and tools
- Current unencumbered Oregon RN license
- Certification in Basic Life Support
