Service Areas: Multiple service territories within New Hampshire.

 Responsibilities

• Admission to HHHC's homecare division.
• Assessing the patient and following a comprehensive plan of care for nursing.
• Maintaining current and accurate patient records.
• Functioning independently in a home setting with minimal supervision.
• Maintaining relationships and communication with patients, families, and care team.

Qualifications

• Will consider associate degree RN with relevant experience who have progressed in their career in LNA, LPN role with homecare experience.
• Registered as Registered Nurse with New Hampshire Board of Nursing.

 Preferred Qualifications

• BSN Degree.
• One to two years acute care experience with med-surg, IV skills, COPD, CHF, and wound care.
• Experience with EMR, we use Netsmart (formerly Allscripts). We will be moving to EPIC in the future!

Required

• Registered as a RN with New Hampshire’s Board of Nursing.
• CPR Certification.
• Satisfactory criminal record.
• Satisfactory motor vehicle record.
