- Identify resident needs and concerns and report to Charge Nurse.
- Conduct resident rounds daily and initiate corrective action as necessary and/or seek assistance of licensed nurse.
- Attend all in-service classes as assigned and complete assignments.
- Has successfully completed a CNA program as required by state and/or federal regulations.
- Assume personal responsibility for following facility procedures related to control of equipment and supplies within the unit.
- Document incidents and/or unusual problems according to established facility procedures.
- Participate in the development of an individualized plan of care for assigned residents.
- Record each treatment provided, refusal of treatment, reason(s) treatment could not be provided and progress notes as required.
- Maintain confidentiality of resident and facility information.
- Report changes in resident condition immediately to licensed nurse.
- Answer call lights promptly.
- Report for work in a clean, neat uniform and consistently present an appropriate professional appearance.
- Consistently work cooperatively with residents, charge nurse, treatment nurses, restorative nurses, nursing assistants, physicians, families, consultant personnel and ancillary providers.
- Accept assigned duties in a cooperative manner.
- Follow infection control procedures.
- Follow Resident Rights policies at all times.
- Assume accountability for all data contained in the employee handbook.
- Perform other related duties in a cooperative manner.
- Agree not to disclose assigned user ID and password for accessing resident/facility information. Promptly report suspected or known violations of such disclosure to the Administrator.
- Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
- Report any known or suspected unauthorized attempt to access facility's information system
- Report suspected or known incidents or fraud related to false billing, cost reports, kickbacks, etc.
- Report suspected resident abuse immediately to the Administrator.
- CNA license.
- Skilled nursing and rehabilitation environment experience
