BASIC FUNCTION: Oversee the daily operation of assigned homes, supervise direct support staff, and ensure the delivery of agency and generic services to assigned individuals.
QUALIFICATIONS:
Associate's degree or equivalent schooling in a related field and at least two years residential as well as supervisory experience.
OR
High school diploma and five years of residential experience and demonstration of leadership abilities.
RESPONSIBILITIES
- Work with the Human Resources department in the interviewing and hiring of new staff for assigned home(s) as needed.
- Approve, develop and implement assigned staff's PTO using the prescribed method.
- Conduct on site training with newly hired staff using prescribed guidelines and values.
- Update assigned home's staff schedule as prescribed through the use of the agency's EVV system.
- Complete performance evaluations on new staff working within 3 months of employment and annually thereafter using individuals' input as well as first hand observations of employee's job skills and performance.
- Carry out disciplinary actions as prescribed by agency guidelines under the supervision of the appropriate program director or designee.
- Serve as on call support for assigned homes via agency provided cellular phone and participate in on call weekend rotation as scheduled.
- ensure the implementation of all procedures involved with the dispatching and maintenance of assigned agency vehicles.
- ensure that all assigned homes are clean and neatly organized.
- Monitor assigned individuals' medication supply and ensure consistent supply on hand. In addition, review MARs on a weekly basis to ensure proper administration and documentation is being completed correctly.
- Complete assigned individuals' medication assessments and all paperwork required for newly prescribed medications.
- Ensure upkeep of all assigned homes and alert administrative staff of any furnishing, renovation or maintenance needs.
- Review assigned individuals' financial documentation on at least a weekly basis. This includes personal and grocery money and ledgers as well as ensuring that utility and other bills are paid on a timely basis. In addition, reconcile financial ledgers with individuals' bank statements on a monthly basis.
- Conduct staff meetings on a regular basis (at least monthly).
- Assist individuals with arrangements for transportation via agency van and/or ACCESS. Complete and submit individuals' monthly expense sheets for ACCESS trips specified by funding source for reimbursement.
- Attend assigned individuals' ISP, ISP annual update and all ISP revision meetings. Assist each participant to express their wants and desires to members of plan committee.
- Schedule and accompany assigned individuals to medical/dental/any other appointments, ensure that immunizations, written recommendations of physicians and therapists and all medical follow-ups are completed as directed.
- Responsible to ensure that regulatory requirements regarding assigned individuals' information binders are in compliance (medical appointments, social/recreation logs, MARS, etc.).
- Coordinate community activities with/for individuals and ensure appropriate staff support is in place.
- Fill in vacant direct care shifts in emergency situations or staff shortages.
- Provide guidance and support to staff and individuals in assigned homes.
- Work with Quality Compliance and Privacy Coordinator to ensure compliance with relevant funding source regulations while advocating for the rights of individuals to live as regular citizens.
- Report critical incidents to individual on call within 24 hours of the incident and provide all necessary information.
- Acquire 24 hours of relevant training including required trainings during the designated training.
- Other duties as assigned.
