Housing Works fights for funding and legislation to ensure that all people living with HIV/AIDS have access to quality housing, healthcare, HIV prevention, and treatment, among other lifesaving services.

This position is included in a bargaining unit of Housing Works' New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union ("RWDSU"). Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a "CBA") between Housing Works and the RWDSU

Compensation Range: $20/hr - $22/hr commensurate with experience

Overview:

Summary Description The Housing Case Manager (HCM) is responsible for case management and supportive care coordination for assigned clients and their households.

Responsibilities:

  • Assess client's daily living skills and as needed provide direct support.
  • Monitor medical condition(s) of clients and participate in care coordination and case conferencing (both internal and external).
  • Track any hospitalization or in-patient stay by clients.
  • Provide referrals and follow-ups when necessary for primary care, substance use, nutrition, clothing, mental health, financial management, and dental.
  • Utilizing the vast array of internal services available to clients, look to connect (and coordinate) with other departments including: ADHC, Health Services, OASAS, Health Homes, Behavioral Health, Community Partnerships, Legal, and Job-Training.
  • Help new clients move into their apartments. Assist clients with their transition from homelessness to stable housing.
  • In collaboration with the client, develop strength based individualized goals. Provide constructive feedback on goal participation during scheduled engagement.
  • Assess and discuss financial management with the client, including follow up with the client on any rent obligation, maintaining an active subsidy, consistently making utility payments, and food and nutritional needs.
  • As needed, accompany the client in the community on scheduled appointments.
  • Participate in weekly supervision with your supervisor. Come to meetings prepared.
  • Complete all required reports and paperwork on or before the prescribed deadlines.
  • Demonstrate excellent customer service with residents/clients; staff; vendors; and all other relevant stakeholders.
  • Enthusiastically participate in advocacy and direct actions to end the twin crises of homelessness and HIV/AIDS


Minimum Requirements

  • Bachelor's degree preferred in social work, psychology, counseling, or related field, or three years of relevant experience in a social service setting.
  • Good communication skills, both verbally and written, including the ability to communicate with all levels of management.
  • Must be proficient in Microsoft Outlook and Office (Word, Excel, and PowerPoint).
  • Dependable & flexible.
  • Ability to maintain a high level of confidentiality.
  • Understanding of harm-reduction practices.
  • Cultural competency, especially around the LGBTQI+ community.
