A well-respected senior living company is hiring a Resident Care Coordinator. This position is responsible for the overall implementation, delivery, and coordination of memory care services at the community. They oversee the day-to-day operation of the unit and are responsible for recruiting, training, coaching, counseling and directing the staff in the Memory Care setting in accordance with company policy and procedures as well as current federal, state, and local regulations and standards.
Responsibilities:
- Coordinate care for all residents by creating an environment and flow to the day that matches each resident's physical and cognitive needs.
- With support from social programming, ensure all memory care staff are trained to provide programming and activities to ensure proper engagement (activities) at all times.
- Communicate with community healthcare professionals, including physicians, discharge planners, nurses, and state agencies on the care needs of residents as needed.
- Participate in community's resident assessment program in accordance with company policy and state regulations. Provide input to Healthcare Director regarding Memory Care resident pre-admission assessments and reassessments.
- Coordinate departmental schedule and ensure adequate staffing in accordance with company standards, policies, procedures, budgets, and state law.
- Ensures compliance with all state regulatory requirements including mandatory reporting to state.
- Ensures that all concern and incident reports are completed, filed and that follow-up is conducted appropriately.
- Manages and coordinates the work of all nursing and other Memory Care staff, communicating pertinent resident health and safety information.
- Performs other duties as assigned.
Qualifications:
- Certified Nursing Assistant I & II (REQUIRED)
- Medication Technician/Aide (REQUIRED)
- Knowledge of Alzheimer's and other types of dementia; provide documentation on certifications and related continuing education.
- 2+ years of experience working with residents with dementia.
- Experience in a Senior Living Community.
- Be passionate about providing a high quality of customer service and resident satisfaction.
- Organized, excellent communication, follow-through, able to work as a part of a team and solution orientated.
