Reserve Behavioral Health- Senior Case Worker

The demands put on us by work obligations, family responsibilities, and hectic schedules can make it difficult to seek medical services within the traditional nine-to-five doctor’s office model. Reservist Behavioral Health Senior Case Manager:
The Case manager’s primary responsibility will be to provide case management services to guests, ensuring that all feasible needs are satisfied where possible. They will be responsible for a variety of casework activities, such as meeting with residents and preparing psychosocial evaluations, which include gathering, analyzing, and evaluating data, as well as developing service plans. The ideal candidate is proficient in Microsoft Office and Spanish. The ideal candidate will also come with knowledge and experience of the City, contacts, and resources already at their fingertips. As part of the team, you will be providing MEDRITE services and excellent care for migrant populations. The pay rate for this position is $31 - $37 per hour.

Keys to success:
The capacity to listen to patients speaking Spanish and problem-solve within means is essential for every position on this project. All staff members must be considerate and kind to patients. Team members must be flexible, and solution-focused with attention to detail, and the ability to quickly process and disseminate information to patients, teams, and leaders. Must possess strong supervisory qualities, as well as the capacity to train others. The majority of the patient population speaks Spanish (preferred), but fluency in other languages, including French, Creole, Haitian, Wolof, and ASL, is a benefit.

Responsibilities:
· Train a team of Case Workers
·Setup and teardown/cleanup workstations at day's end.
· Responsible for a variety of casework activities, such as meeting with residents and preparing psychosocial evaluations, which include gathering, analyzing, and evaluating data as well as developing service plans.
· Utilize materials efficiently with minimal waste.
· In charge of informing residents about the facility's rules and regulations as well as providing information about the service plan process.
· Respect and safeguard the confidentiality of guest-related information.
· Responsible for caseload and conducts record-keeping activities as needed, such as case notes and census statistics reports, while ensuring that all documents are kept confidential.
· Maintain a positive working atmosphere.
· Refer residents to appropriate agencies and programs as needed and build rapport and a professional connection with all stakeholders.
· Assist residents in forming connections with outside services. Makes transportation arrangements for relevant appointments.
· Deal with emergencies when they arise and arrange for medical aid.
· Participate in frequent staff meetings and report progress to supervisor.
· Demonstrate the ability to detect priorities in workflow organization. Capable of performing duties autonomously with little direct supervision.
· Demonstrate good and helpful interactions with clients, visitors, contract employees, volunteers, and coworkers.
· Perform additional responsibilities as needed.
· Demonstrate a dedication to creating a supportive work environment and maintain a good attitude at work and toward the job.
· Project a positive image while interacting with the public and clients and is prepared to go above and beyond to contribute to the development of a quality and compassionate shelter/program.
Qualifications:
· Proficiency in Spanish
· NYC contacts, knowledge, and resources at the ready
· Knowledge of HIPAA (Health Insurance Portability and Accountability Act)
· Outstanding writing and verbal communication abilities.
· Strong organizational abilities as well as the capacity to multitask.
· Bachelor’s degree

Salary: $31.00

** Must be able to stand for extended periods and lift up to 50lbs.
