Remote Care Coordinator

· Remote/WFH

· Monday – Friday

· Salary: $18-20 Dollars/Per Hour

This is not your typical in-office Medical Assistant position. If you are seeking a customary provider office setting to assist providers during exams and collect lab specimens, this is not the job for you. We are looking for a tech-savvy, motivated Certified Medical Assistant who likes helping people. You will play a vital role in this new model of patient care delivery, by closely monitoring important health data of our patient population and communicating with, and caring for, patients telephonically and on our digital care platform. You will have a fundamental role in direct patient care. You can make a difference in the patients’ care by connecting with them monthly to provide support and assist the patient to take charge of their health. Use your medical experience and education to build rapport with our patients and help coordinate their healthcare. We will provide training on the product, program, and proper protocol.

The ideal teammate would be:
An extension of the provider to help close the gaps in your patient’s care. As part of our amazing team of clinical staff, you will build a rapport with your patients through monthly calls as you support them through services such as Chronic Care Management (CCM) and Remote Physiological Monitoring (RPM). Furthermore, you will empower patients to meet their goals, guide them in their healthcare journey and advocate for their needs.

The ideal teammate would be able to perform the following:

  • Manage and support patients' healthcare needs via phone conversations
  • Ensure that daily operational goals and quality metrics are met within standards set forth by RemetricHealth
  • Support care management and care coordination services via our software platform
  • Maintain frequent contact with patients to provide care coordination, support, and manage compliance with the care management programs to increase positive outcomes

Qualifications:

  • Certified Medical Assistant (Current Certification)
  • Ability to stay flexible and quickly adapt to new processes, software changes, and priorities
  • Knowledge of healthcare terms, conditions, roles, and basic care principles
  • A quiet, uninterrupted dedicated workspace ensuring HIPAA compliance is met as well as reliable high-speed internet.
  • An ability to develop and build trusting relationships with patients and partner providers
  • Interest in operating in a new, exciting clinical program and become an integral player in the development of processes and best practices in caring for RPM & CCM patients
  • Must be an empathetic and active listener with the ability to thrive independently in a fast-paced environment
  • Experience performing virtual visits with patients and telephonic care management is a plus
  • Technically savvy, able to help our patients with basic troubleshooting needs with our mobile application and bluetooth enable devices
  • Strong verbal and written communication skills

Salary: $18.00 - $20.00 per hour
