Job Description
Purpose:
Under professional supervision and as a member of an interdisciplinary team, provides rehabilitation and vocational counseling to patients enrolled in the Medically Supervised Outpatient program.
Specific Duties and Responsibilities:
Performs some or all of the following duties:
- Interviews, orients and assesses patient vocational and educational needs.
- Provides individual and/or group counseling to patients in terms of their education and occupation, socio-economic status, medical prognosis and test results.
- Maintains appropriate records.
- Secures cooperation of organizations and employers in the rehabilitation training or placement of patients.
- Attends clinical team meetings to discuss appropriate patient care activities.
- Represents agency in meetings with employers or rehabilitation training agencies as appropriate.
- Maintains accurate, confidential, and legible rehabilitation records on each patient according to agency requirements.
- Assists patient in resume preparation and education.
- Conducts weekly job club for patients work-ready.
- Is knowledgeable of hospital and departmental policies and procedures, and demonstrates the ability to perform his/her duties in accordance.
- Maintains a caseload of outpatient SUD clients.
- Is courteous and professional interacting with patients, colleagues and the public.
Minimum Qualifications
1. A Masters Degree in vocational rehabilitation issued after completion of a two year course in an accredited college.
2. A Masters Degree or 30 credits of graduate study in vocational guidance, vocational rehabilitation, or related fields and one year of full-time experience in vocational guidance, medical or correctional rehabilitation, or
3. A satisfactory equivalent.
