FLSA Status: Non-Exempt JOB TITLE: OCCUPATIONAL THERAPIST-OTR

Department: Rehabilitation

Reports to: Director of Rehabilitation or

Supervisor of Occupational Therapy

Supervises: Certified Occupational Therapy Assistants (C.O.T.A.)

Rehabilitation Aide

This job description, defining qualification, primary and secondary functions, responsibilities and duties is intended as a guideline for the employee and manager. The duties listed here are not considered to be all inclusive. The employee in this position performs these and other duties as required although not listed which may be assigned by his supervisor or facility Administrator.

ESSENTIAL FUNCTIONS:

Conducts an OT evaluation upon receiving physician’s order.

Develops treatment plan based on initial evaluation.

Documents appropriate changes in treatment plan according to patient’s performance.

Schedules and provides direct patient’s care utilizing appropriate therapeutic techniques.

Instructs patient and/or patient’s family members in home program.

Complies with federal, state, JCAHO standards and local regulations related to Therapy service.

Supervises activities of professional and non-professional rehab personnel.

Maintains records pertinent to resident treatment:

A. MD order of eval

B. Clarification order

C. Evaluation report

D. Daily treatment log

E. Weekly summary

F. 14 day physician notification

G. Discharge summaries

H. Completes Medicare forms appropriately

I. Resident care plans

J. MDS - Minimum Data Set

Assumes responsibility for reading and following all documentation guidelines in the Documentation Manual.

Maintains 75% - 80% productivity level.

Demonstrates consistent ability to manage rehab service problems, emergency situations and initiates appropriate action.

Assumes responsibility for the development of Occupational Therapy objectives in conjunction with Director of Rehab.

Communicates with nursing and physician regarding changes or modification in treatment plan or patient status.

Participates in patient care rounds, rehabilitation team conferences, conferences with physicians, members of the physical therapy department and/or rehabilitation team, as necessary regarding problems, progress and needs of patient.

Studies current trends and new developments in occupational therapy treatment by survey of current literature and attendance of professional meetings and conferences.

Observes Resident’s Rights policies at all times.

Actively involved in providing inservice education to all personnel.

Transports patient to and from the therapy department.

Demonstrates knowledge and understanding of the impact of disability upon an individual's, as well as families' physical, emotional, social, economic, and vocational status, taking into consideration age-related issues, and developmental levels over the life span (i.e., the adolescents' need to develop their own identity and cope with life changes; and the geriatrics' need to maintain their integrity and independence and adjust to the physiological aspects of aging), and meets competency requirements for the population served (adolescent through geriatric), as applicable to the job description.

Safety & Health Requirements:

Identifies safety hazards and initiates corrective action.

Follows all facility safety policies and procedures.

Observes infection control procedures standards.

Professional Development Requirements:

Consistently demonstrates responsibility, accountability and dependability.

Consistently works cooperatively and maintains effective communication with facility administration, nursing personnel, ancillary personnel, all facility departments and consultants.

Comes to work clean and neat and consistently presents a professional appearance.

Accepts assigned duties in a cooperative manner.

Consistently maintains awareness of changes in rules and regulations effecting facility operations.

NON-ESSENTIAL FUNCTIONS:

Assists in preparation of employee performance evaluations and recommendations for salary increases, promotions or terminations.

Counsels and provides input to Director regarding disciplinary process.

Assists in hiring new personnel.

Trains and evaluates new personnel.

Participates in the facility CQI Program.

Performs other duties as assigned by the Director of Rehab.

Conducts department tours for interested groups and participates in marketing.

Prerequisites:

Bachelor of Science Degree in Occupational Therapy.

Current registration with American Occupational Therapy Association preferred.

Current CPR Certification required.

Valid state license in Occupational Therapy necessary.

Experience preferred, but not necessary.

Knowledge of growth and development across the life span required.

Reasoning Development Level 6

Mathematical Development Level 3

Language Development Level 4
