The Visit Nurse will visit patients in between case management visits to provide patient care needs. The Visit Nurse will provide patient care with an Interdisciplinary Team of professionals.

Position Requirements:

• ADN required, Bachelor preferred
• Must have RN license in WI and IL. If only license in one state must obtain other upon hire.
• Strong ability to work collaborative.
