registered Nurses -Fishkill Home Care has been servicing the residents of Dutchess County since 1982. We offer a team of experienced and dedicated Nurse Case Managers, who work together with patients, families, doctors and health care facilities to provide a comprehensive home care program for each client. The primary purpose of this job is to provide skilled nursing care and prescribe treatments in patients homes.

Ø The RN will service the following designated counties - Dutchess and Putnam Counties.

Ø WE are Paying a $5,000 signing bonus for this position (to be paid upon successful completion of 90 day probationary period)

Responsibilities and Duties

• Responsible for continuous review of all aspects of every patient on his/her caseload to include: appropriate utilization of services; ensuring continued skilled need; monitoring of home bound status; review of documentation in the medical record; maintenance of interdisciplinary communication and discharge planning.
• Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Admits to appropriate level of service/ care.
• Uses health assessment data to determine nursing diagnosis.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Provide medication management and education.
• Regularly re-evaluates patient nursing needs.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan.
• Manage, evaluate and coordinate care plan implementation with all related disciplines.

Specific Requirements:

• NYS Licensed Registered Nurse (RN)
• One year of experience required
• Experience with Acute Care preferred
• Must possess the ability to deal tactfully with personnel, clients, family members, government agencies/personnel and the general public.
• Must have a valid Drivers License.
• Must have patience, enthusiasm and a cheerful disposition.
