- Provide direct patient care and services
- Provide wound care, medication teaching and self management techniques
- Facilitate patient and family independence in managing their comorbidities
- Promote wellness and goals and interventions to attain goals of care
- Electronic documentation
- Experience in Home Health Care a plus, but 1-year minimum Nursing Experience Required
