The Part-Time Registered Nurse will provide services to sick/recovering individuals in accordance with agency patient care and general medical policies plan of care under the state of Connecticut.
- Completes the initial health assessment for individual clients as per Agency guidelines.
- Assess client needs and home environment.
- Develops and implements individual plan of care and make necessary revisions.
- Makes recommendations to the doctor about potential changes to the plan of care at home.
- Monitor and document clients progress
- Maintain opening communication between the client, caregivers and providers
- Counsels the patient and family/significant other in regard to nursing and other identified needs.
- Supervise certified home health aides/personal care assistant caregiver under the Agency
- Performs therapeutic treatments administered by the physician, with strict aseptic techniques
- Manage client medication
- Monitor vital signs and keeping an accurate record
- Help patients regain their physical independence
- Encourage patients to help themselves within their own capabilities in performing activities of daily living
- Display a high regard for client confidentiality pertaining to client information (HIPPA)
- Communicate with client's doctor nurse, and family requested by the client and in compliance with HIPPA guidelines
- Complies with all Eprine Home Care policies and procedures and uphold the high expectations required
- Abide by administrative policies in relation to scheduling availability, nursing coverage and any other responsibilities
- Maintains a continuing knowledge of competencies related to the nursing profession by participating in in-services and educational programs, workshops, conferences, and professional organizations
- Makes supervisory visits in the patient’s home or another site where the patient is receiving services and regularly evaluates the assigned staff’s performance of the nursing services.
- Provides and documents training to the patient’s caregiver or caregivers and the individual or individuals providing backup
Qualifications
- Graduate of an accredited school of nursing.
- Currently licensed and registered to practice a Registered Nurse in the State of Connecticut
- Experience as an RN with one of the following:
· One (1) year in an acute care setting within the last two years
· One (1) year home health care within the last two years
· Graduate of an accredited RN baccalaureate program within the last year.
- Comprehensive knowledge of general nursing theory and practice.
- Ability to work an integrated health care system
- Verbal and written communication skills
- Demonstrates good interpersonal skills, creativity and flexibility
- Demonstrates ability to take initiative and make decisions independently as necessary.
