MUST:
Registered Nurse in the state of Maryland
One year of experience as a Registered Nurse
Ability to give injections if needed
Knowledge of Patient Centered Nursing Care
Strong clinical skills and knowledge
Experience with documenting in Electronic Medical Records and strong computer skills
Ability to triage patients in person and by phone
Strong ability to provide patient education
Bachelor's Degree in Nursing
CPR certified
DUTIES:
The Duties of the Registered Nurse-Care Manager include but are not limited to the following:
Provide patient nursing care in the ambulatory setting
Perform OBGYN exams and procedures
Perform health screens on patients and employees
Administer vaccinations
Provide direct nursing to patients
Perform triage and provide support by phone
Collect and analyzes patient data
Document phone calls
Create and develop care plans for patients
Document care plans and interventions
Evaluate patient needs
Facilitate nursing visits and provide patient education regarding available treatments for chronic diseases
Provide outreach to patients
Assist with transition care management
Assess patients, assist with medication administration and assist with self-care
Triage patients in walk in clinic
