Responsibilities:
- Provide direct patient care, including assessment, planning, implementation, and evaluation of patient care needs
- Administer medications and treatments as prescribed by the physician
- Monitor, record, and report symptoms or changes in patient's conditions
- Collaborate with interdisciplinary healthcare team members to provide comprehensive care
- Educate patients and their families on healthcare procedures, medications, and self-care techniques
- Maintain accurate and detailed medical records and documentation
- Adhere to infection control policies and protocols
- Follow hospice policies and procedures
Experience:
- Minimum of 2 years of experience as a Registered Nurse in a clinical and hospice setting
- Proficiency in pain management techniques and end-of-life symptom management
- Experience in hospital settings is a plus
- Must be familiar with the use of HospiceMD (comprehensive assessment, supervisory visits, POC, and ordering medication)
Qualifications:
- Valid Registered Nurse license in the state of California
- BLS certification required; ACLS certification preferred
- Strong communication and interpersonal skills
- Ability to work effectively with hospice patients
- Excellent critical thinking and problem-solving abilities
