Responsibilities Comprehensive assessments on all residents Coordination of services with case workers, MD/NP, and other interdisciplinary team members On-call responsibilities for clinical concerns, triage and staffing Collaborate with residents, families, administration, and other staff members to promote individualized care and services.

Pay: $80,000.00 - $86,000.00 per year
