Job Description
The Rehabilitation Hospital of Indiana (RHI) is the largest inpatient rehabilitation facility in the Indianapolis area. RHI provides inpatient and outpatient rehabilitation services to an extensive variety of neurological and medical populations, including individuals with traumatic brain injury, cerebrovascular accidents, neoplasms, neurodegenerative disorders, spinal cord injuries, organ transplants, and many other conditions. We serve patients aged 16 and older.
SUMMARY/PURPOSE OF JOB
This clinical practitioner has a basic knowledge of pathophysiology, the aging process, and the use of the nursing process in the rehabilitation setting. The incumbent personally administers nursing care to the patients and/or plans, directs and coordinates the nursing care of a group of mid-adolescents, adults and geriatric physical medicine rehabilitation patients from age 15 and up via the nursing process and in collaboration with other nursing resources and the health team. The incumbent also instructs patients and families in procedures and health care practices and makes referrals for continuing care after discharge.
Understands and exhibits the principals of service excellence, leads by example, and mentors' others in the performance of their assigned duties. Additionally, understands that every employee is responsible for exhibiting exceptional customer service by answering call lights on the nursing unit and if not able to assist personally, will inform the appropriate employee regarding the patient's need.
Understands the role of employee's individual position regarding HIPAA requirements and level of access to Protected Health Information as defined in HIPAA hospital-wide policies.
Understands it is the responsibility of every RHI staff member to provide patient safety as a priority response according to RHI evidence-based practices. Encourages patients to be actively involved in their own care as patient safety strategy.
PRIMARY JOB DUTIES
Performs initial and ongoing assessment for the mid-teen to adult physical medicine rehab population served; from adolescent age upward, assesses physical, psychological, social, age dependent changes, cultural, environmental, spiritual and economic needs as well as discharge needs. Establishes a compassionate environment by providing emotional, spiritual and psychological support to patients, families and friends.
Assures quality of care by adhering to therapeutic standards, measuring health outcomes against patient care goals and standards and making necessary and immediate adjustments to changes in patient conditions.
Promotes patient's independence by establishing care goals; teaching patients, friends and families to understand condition, medications, answering questions. Develops individualized care plans, which include nursing diagnosis, long and short-term goals and specific nursing interventions congruent with assessment data.
Maintains a cooperative relationship among health care team members by communicating information via bedside handoff, use of white boards, and handoff MAR and task list check. Responds to requests and builds rapport by participating in team problem solving methods.
Demonstrates the ability to effectively take on charge responsibilities, including the making of fair and equitable assignments, holding staff accountable for proper handoff and rounding, and responding to both emergent and non-emergent changes in a patient's condition with confidence, seeking out the assistance of other medical team members as indicated.
The incumbent completes formal ongoing competency evaluation successfully, maintains and demonstrates an acceptable level of clinical competency in the care of the mid adolescent, adult and geriatric physical medicine rehabilitation population served from age 16 upward.
Participates in continuous quality improvement/ program evaluation activities and observes measures to ensure that hospital, Joint Commission, CARF, and other quality standards are met.
Uses appropriate technique in performing duties to avoid or minimize accidents and injury related to unsafe practice. Proactively seeks information and resources, including the Environment of Care Manual, to aid in safe performance of duties to contribute to the RHI culture of safety.
Employee is focused on providing customer satisfaction (including, but not limited to, patients, co-workers, visitors, vendors and physicians) as a primary goal/outcome of all interactions; places customer first in the performance of job duties and responds to needs in a timely manner as evidenced by the absence of legitimate customer complaints.

ESSENTIAL JOB FUNCTIONS
The incumbent could cause death or other negative outcomes directly by administering wrong medication or treatment to patient, by failing to give appropriate attention to patient complaints or requests or by failing to use equipment correctly. Failure to observe and report changes inpatient suspected of infection, etc. could cause patient discomfort or death. Failure to carry through or ensure proper follow-through of patient discharge plans, continuity of care plans, etc., could result in legal and financial complications for RHI. A careless, tactless, and/or unsympathetic manner with patients, relatives, visitors, medical staff, etc., could injure RHI's image. Poor judgment in handling employee problems could create feelings of ill will and dissatisfaction among employees.
QUALIFICATIONS
KNOWLEDGE, SKILLS, ABILITIES
This position requires knowledge of the qualifications, duties and responsibilities of a professional nurse and responsibilities of non-professional personnel as acquired during clinical nursing experience at an approved School of Nursing. Requires an ability to assess the patients' needs (acute care, rehab, and age dependent), and to plan, direct and coordinate nursing patient care area, and an ability to instruct other personnel and the patient and family concerning patient care and health practices. Must maintain an awareness and understanding of legal implications of all patient care activities, own practice and of employee relationships and activity.
The incumbent demonstrates appropriate knowledge of discharge planning and is aware of contacts and/or community resources available to patient and families. Must display the ability to preserve patient dignity, privacy, and confidentiality. Must be able to communicate and interact with patients, families, hospital personnel and preserve confidentiality. Must be able to direct and supervise others in providing patient care activities. Bilingual capability (Spanish/English) strongly preferred.
EXPERIENCE
This position requires experience gained through an approved School of Nursing with an appropriate period of orientation for the performance of nursing care. Direct patient care positions require direct patient care experience in preceding five years. Incumbent must successfully complete nursing orientation objectives.
SPECIAL TRAINING, LICENSURE, CREDENTIALING
This position requires current State of Indiana licensure as a Registered Nurse. Must receive CPR training in accordance with American Heart Association Standards resulting in receipt of course completion card upon hire and maintain current thereafter.
EDUCATION
School of Nursing graduate is required, Bachelor of Nursing degree is encouraged.
