Job Description
Serves as a point of contact between the patient, their family members and the care team to assist with coordination of patient's care.
Manages the transition of patients across the continuum of care, or within a specific clinical setting.
Performs data management function for the program.
Supports physician conferences, such as tumor boards, related to the service.
Works with referring offices to coordinate follow up testing and care.
Performs departmental quality assessments and leads program initiatives.
Performs departmental quality assessments and leads program initiatives.
Requires an associate's degree and is certified as a Registered Nurse Prefer Bachelors degree in Nursing. Familiar with standard concepts, practices, and procedures within a particular field.
BLS certification required or a radiology registry.
INDPR
