The role you'll contribute:
The Admissions Nurse is responsible for responding to and visiting with patients who have been referred for hospice consultation and or admission to AdventHealth Hospice Care. The Admissions Nurse assesses the appropriateness/qualifications of the patient for Hospice services and informs the patients, caregivers, and family members of the hospice services available with the organization. In addition, the Admissions Nurse is responsible for visiting current AdventHealth Hospice Care patients who have been admitted to an AdventHealth hospital.
The value you'll bring to the team:
  • Provides information and available resources to patients/family/caregiver for admittance onto Hospice services.
  • Provides referrals/support for follow-up care for patients who are uninsured or underinsured.
  • Travel to resident site or hospital room of patient who has been referred to Hospice.
  • Partners with the hospital Case Management staff member to meet the needs of the patients and hospital staff to gain new referrals.
  • Assesses the patient and family's emotional condition as to readiness for hospice. Answers questions and responds to concerns to encourage admission onto the Hospice program if conditions are justified.
  • Coordinates efforts for patient treatment and promote quality improvement and educational efforts.
  • Obtains required signatures and generates electronic and or paper documents as required to comply with regulations for reimbursement.
  • Assesses the patient's physical condition, medical needs, current medications, and needs for equipment.
  • Determines patient's primary physician, personal information, and special needs.
  • Obtain orders.
  • Appropriately evaluates hospice eligible patients in a timely manner. Obtains approval from Hospice physician and the patient's primary/attending care physician to admit client to program.
  • Communicates with the patient's primary/attending physician with notification regarding admission of patient and care plan initiation with review.
  • Promptly updates Hospice Intake office/sources on the status of admissions.
  • Review referral documents/information prior to consultation visit.
  • Arranges for transportation of patient as needed.
  • Communicates effectively and timely with the Home Medical Equipment (HME) and the Pharmacist regarding patient-related equipment and medications, as needed.
  • Travels to hospital room of current hospice patient who has been admitted to assigned hospital(s).
  • Notify Care Team of admissions to enable care to be continued by others.
  • Participates in staff meetings, department meetings, in services and other related activities as scheduled.
  • Collaborates with hospital Case Management, Physicians and Nursing Staff to facilitate discharge from the hospital to the appropriate level of care, as needed.
  • Builds relationships with hospital staff to ensure that Hospice is meeting the needs of hospital staff and patients and families.
  • Performs all other duties as deemed necessary by the Director of Operations or designee.

Qualifications
The expertise and experiences you'll need to succeed:
EDUCATION AND EXPERIENCE REQUIRED:
• Diploma from an accredited school/college of nursing required.
• BSN Degree within 3 years of employment.
• Minimum of 1-year Registered Nurse experience in Hospice and/or Acute Care Nursing experience.
EDUCATION AND EXPERIENCE PREFERRED:
• Bachelor of Science in Nursing.
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
• BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date.
Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date.
• Maintain a current, valid Illinois Driver's License in good standing with a safe driving record and a reliable vehicle.
• Maintain valid personal automobile insurance.
LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:
• Certified Hospice and Palliative Nursing certification
