The Registered Nurse (RN) Transplant coordinator works within an interdisciplinary team to coordinate care for a defined population of CART patients. The RN Transplant coordinator is the hub of patients care and interacts/communicates with multiple teams to coordinate care for patients from the CART referral to post infusion. The RN Transplant Coordinator helps support the patient and caregiver along their CART journey in a holistic, patient-centered approach across settings spanning the continuum of care, with an emphasis on assisting patients to seek an appropriate level of service when needed and reducing the overall cost of care. The RN Transplant coordinator will function within the Mayo Clinic Nursing Professional Practice Model, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for an assigned population of patients. The ANA Nursing: Scope and Standards of Practice provide a basis for the practice of the RN. The American Academy of Ambulatory Care Nursing Scope and Standards of Practice for Registered Nurses in Care Coordination and Transition Management (2016) are reflected.

This hybrid position will also work a job 2 as a hematology Registered Nurse (RN) that provides direct patient care to patients with malignant and non-malignant hematologic diseases. Direct care responsibilities include, but are not limited to symptom assessment and management, patient education, and care coordination. The hematology RN serves a fundamental role of the multidisciplinary care team through daily collaboration. The hematology RN may be required to cover a variety of disease groups, such as Cellular Therapy and dysproteinemia. Individuals who are interested in this role will be required to obtain the adult Oncology Certified Nurse (OCN) credentials within three years of employment.

The Transplant Coordinator Nurse is accountable for the coordination of all aspects of care for the transplant patient population. Assesses and plans in collaboration with the patient, caregiver, and multidisciplinary healthcare team to meet needs across the care continuum. Assesses data reflective of the patient's status and appropriately interprets information relative to the patient's age-specific needs. Provides patient and family education on a consistent basis. Provides leadership through activities such as community involvement, committee work, and participation in quality improvement and research. The American Nurses Association (ANA) Nursing: Scope and Standards of Practice, the ANA Code of Ethics for Nurses, the United National Organ Standard's (UNOS), and the Foundation for the Accreditation of Cellular Therapy (FACT) guidelines provide the basis for practice.
This role is eligible for TN sponsorship. Holders of F1 status eligible for post-completion OPT are welcome to apply. This role is NOT eligible to participate in the F-1 STEM OPT extension program. Successful sponsorship will require state licensure and completion of the VisaScreen or equivalent certification.


Qualifications

Graduate of a baccalaureate Accreditation Commission for Education in Nursing (ACEN) or Commission on Collegiate Nursing Education (CCNE) accredited nursing program. Minimum 1 year of clinical RN experience required, 3 years preferred.

Additional Qualifications:

BSN required. Must become certified within 3 years. Must be currently working in direct patient care. Excellent communication skills (verbal and written). Experience working in a team environment. Ability to adapt to unpredictable situations within the work setting. Demonstrated leadership, effective communicator, and excellent critical thinking skills.

Current RN license by applicable state requirements.

Basic Cardiac Life Support for healthcare providers- BCLS – certificate of course completion required and must not expire within 3 months after start date. A scanned copy of the certificate of completion or transcripts, including your full name, must be attached to your profile to be considered.

We will only accept certificates of course completion from the following providers:

  • American Heart Association (Title: BLS for Healthcare Providers or BLS Provider)
  • Red Cross (Title: CPR/AED for Professional Rescuers and Health Care Providers or BLS for Healthcare Providers)
  • Military Training Network

Additional specialty certification/training as required by the work area.
