Key Responsibilities: Mission Control RN
As a Mission Control RN, you will be responsible for providing top-notch patient care to diverse populations within the framework of the YNHHS Home Hospital Model. Your key responsibilities will include:
- Utilizing monitoring, assessment, planning, intervention, and critical thinking skills to care for acute care patient populations in the home.
- Navigating patients and families across the continuum of YNHHS Home Hospital phases of care.
- Monitoring and responding to biometric data per YNHHS Home Hospital protocols.
- Conducting home and audio/visual patient care visits as directed by the care team and/or protocol.
- Ensuring efficient implementation of all interventions in the patient's care plan with a patient-centered approach.
- Collaborating with the YNHHS Home Hospital team and supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines, and documentation systems to support safe, reliable, high-quality, evidence-based care.
Qualifications:
- Education: Graduation from an accredited School of Nursing. All newly hired Registered Nurses must have current BCLS certification or obtain it within one month of hire.
- Experience: Recent 2 years of bedside nursing required. Care Management experience a plus. Excellent communications, interpersonal, coordinative, and organizational skills.
- Accountability: Provides nursing care to an individual or group of patients; recognizes changes in a patient's condition and reports such changes to the Physician and/or Patient Services Manager. Utilizes and manages material resources in a cost-effective manner.
- License: RN license and currently Registered Nurse in the State of Connecticut. All newly hired Registered Nurses must have current BCLS certification or obtain it within one month of hire.
