Key Responsibilities: Clinical Nurse I (Mission Control Field RN)
- Nursing Process:
- Utilize the nursing process of assessment, planning, intervention, and evaluation while maintaining confidentiality and dignity.
- Provide safe, effective, evidence-based care tailored to individual patient and family needs.
- Partner with the care delivery team to coordinate patient care at the unit level, following the principles of patient/family-centered care.
- Clinician Patient Relationship:
- Demonstrate a holistic and caring approach throughout the nurse-patient relationship.
- Clinical Expertise/Problem Solving:
- Collaborate with the multidisciplinary team to support teamwork, unit activities, and positive patient outcomes.
- Collaboration/Teamwork:
- Delegate responsibilities based on an understanding of others' roles, scope of practice, and capabilities.
- Contribute to a positive work environment, respecting cultural and human diversity.
- Practice Development/Positive Impact:
- Successfully complete competency-based orientation.
- Participate in activities to improve the care environment.
- Stay current with staff meeting minutes and support student learning.
- Assume responsibility for continued learning and integration of evidence-based practice.
- Deliver excellence in patient care, complying with all patient safety and regulatory initiatives.
- Leadership:
- Communicate in a calm, professional manner with patients, families, colleagues, and other hospital personnel.
- Use resources in a fiscally sound manner.
- Mission Control Field RN Responsibilities:
- Provide high-quality patient care within the YNHHS Home Hospital Model.
- Utilize monitoring, assessment, planning, intervention, and critical thinking skills for acute care patient populations in the home.
- Navigate patients and families across the continuum of YNHHS Home Hospital phases of care.
- Monitor and respond to biometric data per YNHHS Home Hospital protocols.
- Conduct patient care visits as directed by the care team and/or protocol.
- Ensure efficient implementation of all interventions in the patient's care plan with a patient-centered approach.
- Collaborate with the YNHHS Home Hospital team and supplier partners for safe, reliable, high-quality, evidence-based care.
Qualifications:
- Education: Graduation from an accredited School of Nursing. All newly hired Registered Nurses must have current BCLS certification or must obtain within one month of hire.
- Experience: Recent 3-5 years of acute care nursing experience required. Care Management experience a plus. Excellent communications, interpersonal, coordinative, and organizational skills.
- License: RN License and current registration in the State of Connecticut. All newly hired Registered Nurses must have current BCLS certification or obtain it within one month of hire.
- Must have a valid driver's license for the Mission Control Field RN role.
