Registered Nurse (Rn) - Case Manager - Homecare

Centra Healthcare Solutions is seeking to hire a Registered Nurse (RN) - Home Health Case Manager.

The Registered Nurse (RN) is responsible and accountable for the delivery and coordination of patient care and the application of the nursing process. The RN demonstrates the ability to make clinical judgments and utilize critical thinking in an effective and efficient manner with supervision. As well, the Registered Nurse (RN) will perform according to nursing standards and is accountable in managing patient care and assisting others in the management of patient care. This is a direct hire / 'perm' opportunity.

Minimum Requirements include:

• Recent experience in Case Management working as a Registered Nurse / RN is preferred.
• Develops, delivers/implements, revises, and evaluates individualized patient-centered care plans under the supervision of a physician.
• Provides case management to an assigned group of patients and ancillary staff. Pediatrics and adult experience preferred.
• Makes the initial evaluation visit and regularly reevaluates the patient's nursing needs per state and payor requirements.
• Homecare Homebase charting experience is preferred.
• Reliable transportation to commute throughout the greater Denver metro area.  4 days in the field and 1 day in office for case reviews with Director of Nursing. 
• Current Registered Nurse License within the state of practice.
• Current BLS.
