JOB DESCRIPTION:
This Trauma Case Manager coordinates the care and services of selected trauma patient populations across the continuum of illness; promotes effective utilization and monitoring of health care resources; and assumes a leadership role with the interdisciplinary team to achieve optimal clinical and resource outcomes. He/She also assists the Trauma service with quality of care issues and identifies opportunities for improvement.
EDUCATION:
Current RN licensure or compact licensure recognized by the State of Vermont is required. The University of Vermont Medical Center is moving toward an all Bachelor of Science in Nursing (BSN) workforce. All external candidates hired into, or current employees promoted to, a RN position will be required to obtain their BSN as outlined in the terms of their offer letter. Current internal RNs are strongly encouraged to pursue a BSN to support this initiative.
EXPERIENCE:
Three-five years recent clinical experience in trauma.Two (2) years experience in case management and/or similar role preferred.
