Job Summary

Provides nursing and health care services as identified in a client’s Person Directed Plan (PDP), Individual Plan of Care (IPC) and Implementation Plan (IP).

Essential Job Duties

· Completes a Comprehensive Nursing Assessment (CNA) upon admission to the non-residential program and annually thereafter.

· Complete quarterly focused nursing assessments

· Coordinates the health services of clients on assigned case load.

· Ensures face-to-face visits with clients occur as developed in the individual plan.

· Administer medications & perform procedures as ordered by physician.

· May assist in facilitation of physician appointments for clients and/or HH/CC providers.

· Provides assistance to other nursing departments as able or necessary.

· Assist with “on-call” nursing duties as necessary.

Job Type: Full-time

Pay: $74,000.00 - $75,000.00 per year
