Description 

This Registered Nurse position is assigned to the Psychiatric Services Team. May work in the Medication Assisted Treatment Program (MAT) or clinic program. Works as part of the team to ensure services and resources are coordinated among providers as needed to help individuals served live and function optimally in the community.

Starting Salary: $52,379-Depending on Experience

 Responsibility 

  • Works as part of the team to complete post hospitalization assessments, making sure the individual has medication and follow up appointments as necessary and appropriate.
  • Completes and updates as required assessments of needs and strengths to meet the admission and ongoing provision requirements of the service(s) requested and indicated for the individual. Ensures the proper pre-authorization documentation to qualify for and receive payment for all services rendered are submitted within allowable time frames and maintained in the record. 
  • Manages scheduling and any call list for these referrals. Monitors individuals, making sure that they attend all necessary appointments. If needed, reaches out to the individuals to see what has hindered their participation in services. 
  • Writes effective and measurable Individualized Service Plans (ISP) as required upon admission, as the needs of individuals served change, and at least once annually thereafter. Works to ensure supports and interventions are provided as stated in the ISP. Ensures all assigned ISPs are reviewed at least quarterly. 
  • Completes all essential tasks determined necessary to provide comprehensive psychiatric care to individuals which includes, but is not limited to the following: checking messages on a daily basis and managing all calls; obtaining Prior Authorizations for Medications as needed; completing refill requests in a timely manner; facilitating direct contact between the Psychiatrist and other NRVCS staff as needed during a crisis/emergency situation, covers/backs up other clinics as needed, and assisting in maintenance of the Psychiatric Services waiting list in conjunction with other Psychiatric Team members and the supervisor. 
  • Collaborates with other agencies and NRVCS programs to ensure that all services reflect the individuals' needs and ISPs. Monitors satisfaction with services provided and helps individuals served complete satisfaction surveys as needed. 
  • Assists with crisis resolution and general problem-solving. Ensures services are provided with respect to the dignity of the individuals served as outlined in the State and NRVCS Human Rights Plans. Advocates for services on behalf of individuals served and their families/care givers as needed. Documents and reports suspected violations of Human Rights according to NRVCS policies. 
  • Promotes independence by teaching and modeling life skills such as: personal hygiene, nutrition, understanding of mental and physical health and well-being, methods to access assistance, financial management, coping skills, self-esteem and self-reliance. 
  • Establishes and maintains comprehensive systems for referral and networking of service and resource needs. Works to establish linkages between individuals served and naturally occurring supports in the family and community. 
  • Works with team to ensure individuals' health and safety needs are addressed as needed including medical, dental, psychiatric, psychological, physical therapy, speech therapy, occupational therapy, assistive technology, and environmental modification needs. 
  • Maintains complete and accurate medical records, and completes all administrative documentation (i.e.: PowerTime, incident reports, leave requests, etc.) on time. Stays informed about and follows all NRVCS and funding sources' policies and procedures. 
  • Performs at the expected levels of individual and team productivity. Ensures all records are in compliance with all applicable local, state, and federal requirements as they pertain to DBHDS Licensure Regulations, HIPPA, 42-CFR Part 2, Clinic Options Medicaid, Waiver, State Plan, NRVCS Policies and any other applicable regulations and/or guidelines. 
  • Maintains all communications, written or verbal, with respect and confidentiality and complies with HIPAA, and 42-CFR Part 2, standards and regulations. 
  • Ensures all safety, health, and infection control procedures are properly addressed and monitored. Immediately reports to a supervisor any observed or suspected problems that involve individual or program safety, welfare, and/or other potential policy and procedure violations and completes a NRVCS Incident Report Form documenting each incident within 24 hours. 
  • Strives to promote continuous quality improvement of services through the timely completion of QA activities (i.e.: satisfaction surveys, etc.) and participates in ongoing networking with individuals served, families and/or care givers, other NRVCS programs, and other community agencies and resources as needed. 
  • Ensures all required certifications are kept up to date. Attends/provides other training as required to meet personal professional development and/or program needs. Provides on-site orientation for new staff, students and volunteers as assigned by supervisor. 
  • Correctly administers medications to individuals as prescribed and in accordance with all medication administration regulations. Provides assistance with other medical procedures (i.e.: Injectable medications etc.) as ordered by a physician and approved by supervisor. Completes a Medication Administration Incident Report Form within 24 hours for any problems that involve medications. Assists in training staff in medication and health related topics and assists with Medication Quality Assurance activities as assigned. 
  • Promotes positive communication and collaboration among team members and other community resources needed to provide quality services. Conducts self in a professional manner at all times. Participates in team meetings unless absence is excused, completes proper written communication in Communication Logs, and relays other helpful information as necessary by voice mail and/or e-mail. Serves on designated teams, committees and work groups. 
  • Works with team to ensure agency meets revenue and expense expectations, as outlined in annual budget. 
  • Completes other tasks and duties as assigned by supervisor, service line manager and/or director. 
  • Vivitrol Nurse only: (Employee will initial as part of their job duties) _______ Provides Care Coordination services to all Vivitrol patients and also to clients involved in the legal system as assigned. _______ Collaborates with internal and external staff to ensure a steady flow of referrals for Vivitrol is maintained. _______ Focuses primarily on clients that have been recently incarcerated or involved in the legal system and have an Alcohol and/or Opioid Use diagnosis. _______ Provides back-up coverage for MAT and psychiatric clinics, as needed, with primary focus on clients that have been recently incarcerated or involved in the legal system and have an Alcohol and/or Opioid Use diagnosis. Completes any necessary tasks to ensure that the MAT and/or Psychiatric clinic workflows are supported and maintained. 

Qualifications 

  • Knowledge of the nature of serious mental illness, substance use disorders, intellectual disabilities, including clinical and developmental issues. 
  • Thorough knowledge of the use of medications in the care of the populations served. 
  • Knowledge of services, systems, and programs available in the community including primary health care, support services, eligibility criteria and intake processes, generic community resources, and mental health, intellectual disabilities, and substance use disorder treatment programs; 
  • Knowledge of different types of assessments, including functional assessments, and their uses in service planning; 
  • Knowledge of treatment modalities and intervention techniques, such as behavior supports, independent living skills, training, supportive counseling, family education, crisis intervention, discharge planning, and service coordination; and 
  • Knowledge of federal and state laws, regulations, and local ordinances (examples include Human Rights, Medicaid and Medicare guidelines as they pertain to Waiver, State Plan, Medicare-D, and Clinic Options, Assisted Living Regulations, etc.) 
  • Must possess a valid driver's license (state of Virginia, unless otherwise approved by agency) with an acceptable driving record. Must maintain acceptable record throughout employment. 
  • Must have an acceptable criminal history background and child protective services check upon hire and maintained throughout employment; 
  • Must have acceptable drug screen results upon hire. 

Education/Experience Required

  • Graduate of an approved school of Nursing is required. 
  • Virginia Registered Nursing License is required. 
  • Bachelor's Degree or beyond is preferred.
  • State Certification as a Medication Administration Trainer is helpful as is certification as a CPR and First Aid Instructor.
