PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors’ health so they can live safely in their homes. HopeWest PACE provides home care, access to the day center and clinic, transportation to and from medical appointments, and activities to enhance the lives of patients and their families. This innovative program balances medical, emotional, physical, and social needs of seniors.
The PACE Registered Nurse provides clinical nursing and procedural expertise by telephone or in person, either in the clinic or in the home setting, based on where the need is. This PRN position will provide flexibility while ensuring high quality care is provided to the participants based on their individualized plan of care.
What You Bring:
- Current Colorado Licensure as a Registered Nurse (RN)
- At least 1 year experience working as an RN
- Valid Colorado driver’s license, adequate automobile insurance and access to reliable transportation if working in the community setting
- Ability to work collaboratively and communicate effectively
- Ability to de-escalate a tense or crisis situation

- Competitive pay structure starting at $35.00-37.50 per hour, depending on experience and education
- Plus 10% variable differential
