PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors health so they can live safely in their homes. HopeWest PACE provides home care, access to the day center and clinic, transportation to and from medical appointments, and activities to enhance the lives of patients and their families. This innovative program balances medical, emotional, physical, and social needs of seniors.
The PACE ClinicRegistered Nurse is responsible for providing evidenced based nursing services to participants enrolled in the PACE program. The Clinic RN provides support the participant and their caregiver/family members, as well as assists the participant to maintain or increase their health status through the provision of Clinic services including teaching, counseling, and other skilled interventions.
What You Bring:
- Current Colorado Licensure as a Registered Nurse (RN)
- At least 1 year experience working as an RN
- Valid Colorado drivers license, adequate automobile insurance and access to reliable transportation if working in the community setting
- Ability to work collaboratively and communicate effectively
- Ability to de-escalate a tense or crisis situation
