Job description

The Massachusetts Department of Mental Health (DMH) is seeking an experienced and dynamic nursing professional to serve as the Nursing Facility Transition Manager. The selected candidate directs, manages, and oversees all aspects of the federal Preadmission Screening and Resident Review (PASRR) program and coordination of specialized services and community transitions from nursing facilities for individuals determined by PASRR to meet the criteria for Serious Mental Illness (SMI).   

This is a vital leadership position with oversight for all assessment, service coordination and transition planning for individuals with SMI, including the PASSR program and coordination of specialized of services by Behavioral Health Community Partners (BHCPs) and other Integrated Care Organization care coordination resources. This position manages the coordination and oversight of DMH Transition Case Management Team assigned to nursing facilities to facilitate community transition.


Duties and Responsibilities (these duties are a general summary and not all inclusive):

  • Oversees PASRR screening and resident review processes in partnership with MassHealth and UMass.
  • Manages the process for assessment, coordination, and provision of specialized and transition services across PASRR, BH CP and Integrated Care Organization care coordination resources and DMH services.
  • Supervises the DMH Nursing Facility Specialist.
  • Directs the process for assignment of DMH transition case management for individuals identified for transition/diversion from nursing facilities and provide supervision and support for case management activities.
  • Maximizes community living opportunities for individuals with SMI who are residing in nursing facilities and diversion to the community.
  • Champion DMH's mission and commitment to race, equity, and inclusion results in equitable opportunities for treatment and transition.
  • Ensures strategic leadership in coordinating interagency solutions to complex problems and communication.
  • Partners with DMH inpatient facilities and Areas on transition planning for individuals with complex medical, behavioral and mobility needs.
  • Collaborates with EOEA, EOHHS, MassHealth, MRC and DDS on case reviews and interagency projects for adults with mental illness with complex needs.
  • Designs and implements data analytic tools and reports to perform contract monitoring and performance improvement activities for use by DMH and other stakeholders.
  • Evaluates the work performance of all staff and facilitates ongoing feedback/appraisal.
  • Recommends design and policy changes for assessment, coordination, and provision of specialized and transition services in nursing facilities.
  • Establishes quality and efficiency standards in all areas of responsibility and ensure that these standards are measured and reviewed on an ongoing basis.

 

Special Requirements:

  • Current and valid registration as a professional Registered Nurse under the Massachusetts Board of Registration in Nursing.

 

Preferred Qualifications:

  • Thorough knowledge of Federal PASRR regulations/policies regarding PASRR requirements, timelines, structure, and implementation.
  • Six to eight (6-8) years supervisory experience preferably in a hospital or healthcare setting.
  • Proficient understanding of principles and practices of nursing including psychiatric nursing and theories of mental health and recovery.
  • Knowledge of the principles and practices of management, especially planning, organizing, directing, motivating and decision-making.
  • Ability to understand, support and implement the principles of race, equity, and inclusion.
  • Competence in establishing relationships with community providers and staff members.
  • Capacity to organize, manage and supervise others, including feedback and appraisal.
  • Proven experience in all aspects of quality improvement in a healthcare setting.
  • Strong technology skills and experience with Joint Commission Standards and CMS Regulations.
  • Proficient usage of Microsoft Office products including Word, Excel, Teams, and Outlook.
  • Given the population served, bilingual or multilingual fluency.

 

Please note: Location is flexible, and office can be based in one of the DMH Area Locations.

MINIMUM ENTRANCE REQUIREMENTS:

 

Applicants must have at least (A) five (5) years of full-time or, equivalent part-time, professional, administrative, supervisory, or managerial experience in a particular specialty (i.e. scientific, professional, or technical) and must possess current license and/or registration requirements established for the performance of the position, of which (B) at least one (1) year must have been in a project management, supervisory or managerial capacity or (C) any equivalent combination of the required experience and substitutions below.

 

Substitutions:

 

I. A certificate in a relevant or related field may be substituted for one (1) year of the required (A) experience.

 

II. A Bachelor's degree in a related field may be substituted for two (2) years of the required (A) experience.

 

III. A Graduate degree in a related field may be substituted for three (3) years of the (A) required experience.

 

IV. A Doctorate degree in a related field may be substituted for four (4) years of the (A) required experience.
