Join Acenda's Integrated Health Team as a Registered Nurse with our Nurse-Family Partnership program in Atlantic County.

What you'll do
- Travel to clients' homes and provide home visits to first-time mothers providing education and support through pregnancy and up until the child's 2nd birthday.
- Work to improve pregnancy outcomes, child health and development outcomes, and the economic self-sufficiency of families.
- Build long-term therapeutic relationships with clients and their families.
- Provide community resources as needed per nursing assessment.
- Adheres to the nursing process and the Nurse-Family Partnership model of home visitation.
- Attends and participates in case conferences and team meetings.
- Collaborates with client and family to develop short- and long-term goals.
- Formulates nursing diagnosis based on nursing assessment, client goals and Nurse-Family Partnership outcomes.

What you'll bring to the team
- Bachelors degree in Nursing (BSN) required
- Active NJ Registered Nurse license (in good standing)
- Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record
