Meridian Group Inc is seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as Director of Nursing! We are a newly established Adult Day Care under the Elderly, Blind and Disabled (EBD) Medicaid waiver, dedicated to providing exceptional care for our clients. As a Part-Time RN or LPN, you will play a crucial role in developing individualized care plans, monitoring client health, and fostering friendly and supportive relationships with our clients that encourage self care and healthy lifestyles. Responsibilities:
- Development of Care Plans - conducting assessments of client's health, medical history, and needs while collaborating with other medical experts and client representatives to meet client unique requirements and goals.
- Health Monitoring - Regular monitoring of the on-going medical needs of participants and the supervision of medications while providing ongoing health education to clients regarding wellness and disease prevention.
- Communication - Collaborate with Direct Care Staff and entire Meridian team to maintain open communication with clients and optimize client care outcomes.
- Documentation - Maintain detailed and accurate records for each client, as well as all care activities, assessments, and additions or changes to client care plans.
- Emergency Response - Respond appropriately and immediately to medical emergencies by following established policies and procedures of Meridian Adult Day Care, as well as any first aid and coordination of any medical services necessary.
- Leadership - Supervise all future Certified Nursing Assistants (CNA) and delegate above responsibilities accordingly.
Qualifications:
- Current and valid Colorado RN or LPN license, or valid and current membership in the NLC.
- Previous experience in nursing or other medical fields
- Current CPR and BLS certification
- Ability to build positive relationships with clients and team members.
- Excellent organizational and time-management skills.
- Strong problem solving and critical thinking skills
Schedule:
- 10 Weekly Hours Total throughout Monday - Friday : Two hours daily within the working hours of 9:30am - 2:30pm. The two hour timeframe is up to the discretion of the RN/LPN and can fluctuate if necessary. No evening or weekends will be required.
