*Nightshift position*
Under the supervision of the JTR Nurse Manager primarily assigned to the Journey to Recovery Substance Abuse Detox and Residential Treatment Center. Provides services on the Withdrawal Management Unit (WM), provides nursing services to adult JTR Members with the goals of assisting patients with the Substance Use Disorder Withdrawals to include managing psychiatric crisis, substance abuse withdrawal and medication assisted treatment. (MAT). Works in conjunction with contracted Medical Addictionology Physician of SRPMIC JTR. Assists in all levels of care to include: Withdrawal Management/Detox Unit, Residential Unit and Office Based Opiate Treatment Outpatient Care Facility. This job class is treated as FLSA Non-Exempt.

Examples of Tasks
1. Patient Interaction: Performs JTR nursing duties, including obtaining health histories and performing physical assessments and withdrawal management protocol standards to include assistance with self-administration of medication, medication assisted treatment (MAT) protocols, CIWA Protocols and COW Protocols.
  • Initiates preventative health measures within the scope of practice for a professional Registered Nurse (RN).
  • Evaluates and carries out medical providers' orders for substance abuse patients in JTR WM Unit. Also assists with nursing duties on Crisis Unit and Residential Unit.
  • Initiates appropriate interventions according to health care priorities identified in assessments.
  • Adheres to established policies and procedures and standing orders.
  • Coordinates programs, services and other activities to implement targeted health promotion and disease prevention activities.
  • Maintains patient confidence and protects operations by monitoring confidential information processing.

2. Administration: Assures adherence to established policies, procedures and standing orders.
  • Assists as needed with the coordination of all health related appointments and referrals.
  • Coordinates health care aspect of patients if they transition to another facility, hospital, treatment center or other.
  • Documents accurately, legibly and completely in Electronic Health Record (EHR), patient charts. Includes subjective and objective data, treatment provided, evaluation of treatment effectiveness and patient education provided.
  • Develops and documents nursing care plans and interdisciplinary treatment plans.
  • Establishes and maintains documentation and communication flow between staff at Salt River Clinic
  • Appraises quality and effectiveness of documentation, education materials and equipment periodically and makes recommendations to clinic leadership for change when necessary.
  • Follows through to ensure that agreed upon changes are implemented and maintained.
  • Maintains program data.
  • Prepares and submits required reports as needed.
  • Participates in case staffing as required.

3. Patient Documentation, Care & Treatment Plans: Documents patient charts which includes subjective and objective data, treatment provided, evaluation of treatment effectiveness and patient education.
  • Develops and documents nursing care plans and interdisciplinary treatment plans.
  • Establishes and maintains documentation and communication flow at each program site.
  • Appraises quality and effectiveness of documentation and education materials and equipment as needed.
  • Recommends changes when necessary and follows through to ensure that agreed upon changes are implemented and maintained.

4. Case Management: Participates in case management meetings with all programs servicing assigned clients.
  • Works with families, Community programs and medical personnel to arrange for additional services including convalescent and rehabilitative care as well as medical specialty services.
  • Participates as a member of multi-disciplinary consultation and review teams including case assignments.
  • Attends staff meetings as requested.

5. Medication Monitoring: Monitors efficacy of medication, side effects and drug interactions.
  • Provides medication education to clients.
  • Sets up and monitors medication administration.
  • Coordinates directly with contracted physician of WM Unit and trains all medical and clinical staff on unit to ensure monitoring of all medications.

6. Coordination of Care: Oversees care coordination for individuals and families including coordination of all health-related appointments.
  • Provides referrals for clients with social and emotional problems or any other needs, beyond the scope of the RN, to other community agencies and services for assistance, as needed.
  • Oversees all client transitions to other facilities, programs or homes.

7. Data Collection/Analysis:Collects comprehensive data pertinent to the health status of the population and community served.
  • Maintains and analyzes program data to make suggestions for program enhancements.

  • Prepares and submits required reports as necessary.

8. Health Education/Facilitation:Leads, teaches and facilitates health education and health skills training opportunities for the Community and employees of the Community, including CPR, MAT, diabetes education, and bloodborne pathogens.
  • Collaborates with representatives of the Community, service organizations and HHS professionals in providing for, and promoting, the health of the population.
  • Actively reaches out to all who might benefit from an intervention or service.
  • Identifies and investigates health risks to the Community by working with other health professionals as assigned.

9. Miscellaneous: Performs other duties as assigned to maintain and enhance the health care program and Department of Health and Human Services service operation.
  • Maintains professional knowledge and proficiency in nursing through continuing education, staff meetings, training and conferences, etc.

  • Knowledge of theory, principles and scope of practice for a professional Registered Nurse.
  • Knowledge of federal, state, and agency laws and regulations governing professional nursing practice.
  • Knowledge of health systems.
  • Knowledge of current and emerging issues in Public Health.

  • Knowledge of data collection and analysis.
  • Knowledge of theory and practices of management in nursing services.
  • Knowledge of assessment, preventative and therapy procedures in a correctional setting.

  • Knowledge of medical, public health and social service resources available to Native Americans including, but not limited to, Indian Health Service, Medicare, Medicaid, ALTCS, SSI, etc.
  • Knowledge of public health practices and concerns.
  • Knowledge of history, culture, laws, rules, customs and traditions of the Salt River Pima-MaricopaIndian Community.

  • Skill assessing client needs and developing care plans.
  • Skill observing, assessing and documenting patient behavior and progress.
  • Skills with Opiate Use Disorder/Alcohol MAT Treatment and medically managed withdrawal.
  • Skill in assessing patients in substance use withdrawal or psychiatric crisis.
  • Skills in monitoring patient stability psychiatrically and medically on higher level of care units.
  • Skill with verbal and written communication and ability to communicate effectively with a wide range of people and health professionals.
  • Skill maintaining accurate records and compiling timely reports.
  • Skill performing emergency medical procedures including first aid and Basic Cardiac Life Support.
  • Skill applying professional nursing methods and techniques.
  • Skill applying and interpreting federal, state, and agency laws and regulations governing professional nursing practices.

  • Ability to establish and maintain positive working relationships with colleagues, subordinates, supervisors, health care professionals, Tribal officials and representatives of resource agencies.
  • Ability to work as a member of a team.
  • Ability to incorporate cultural differences, health beliefs and learning styles.
  • Ability to identify and address programmatic problems.

Minimum Qualifications
  • Education and Experience: An Associate Degree or Bachelor's in Science Degree in Nursing from an accredited college or university and certification as a Registered Nurse(RN) required.
  • Arizona Registered Nurse license required.
  • Experience working with Native American Communities preferred.
  • Experience working in a health clinic as a Registered Nurse preferred.

  • Equivalency: Any equivalent combination of education and experience that will allow the applicant to satisfactorily perform the duties of the job may be considered.

  • Underfill Eligibility: An enrolled Community Member whom closely qualifies for the minimum qualifications for a position may be considered for employment under SRPMIC Policy 2-19, Underfill

Special Requirements
  • Employees in, and applicants applying for, jobs providing direct services to children are subject to the "Community Code of Ordinances", Chapter 11, "Minors", Article X. "Investigation of Persons Working With Children".
  • Must have current certification in First Aid and CPR prior to working with patients.
  • May be required to work beyond normal business hours including nights, weekends and holidays.
