PRINCIPAL DUTIES AND ESSENTIAL FUNCTIONS:

Job duties specific to this position:

• Takes responsibility for ensuring that all work outcomes satisfy the HealthAlliance-Clinton Hospital Three Pillars: Finance, Quality, and Service Excellence.

• Demonstrates excellent attendance and actively participates in a variety of meetings and training sessions as required.

• Identifies expected outcomes in a plan individualized to the patient, family and/or caregivers and the situation.
• Develops a plan that prescribes strategies and alternatives to attain expected outcomes.
• Implements the individualized patient plan of care.
• Develops and implements a plan to obtain expected outcomes that are individualized to the patient, family and/or caregivers and the situation.

 JOB KNOWLEDGE AND SKILLS:

• Ability to perform all duties according to American Nurses Association Scope & Standards of Practice for Home Health Nursing professional standards and HAHHH policies and procedures.
• Ability to use a laptop computer. 
• Ability to proficiently utilize or learn to proficiently utilize MS Office applications, e-mail, Point-of-Care software applications, and other wireless applications.
• Ability to perform comprehensive physical and psychosocial assessments.
• Ability to work autonomously.
• Ability to communicate professionally and effectively with a diverse patient population.
• Ability to read, write and speak clearly in English. 

EDUCATION:

• Graduate of an accredited school of nursing required .

LICENSES, CERTIFICATIONS, ETC.:

• Valid and unrestricted licensure as a Registered Nurse in the Commonwealth of Massachusetts required.
• Valid and unrestricted driver’s license recognized by the Commonwealth of Massachusetts and access to a reliable, insured vehicle required. 

EXPERIENCE:

• Experience is preferred but not required
