The Clinic Registered Nurse (RN) is responsible for performing a competent level of nursing care as demonstrated by the critical thinking model known as the nursing process. The standards of practice include assessment, nursing diagnosis, outcomes identification, planning, implementation, coordination of care, teaching and health promotion and evaluation.
Responsible for the provision and facilitation of comprehensive care coordination services and quality outcomes for patients across the continuum of care. Promotes effective utilization of health services, collaborates, and communicates with the healthcare team and patient/caregiver to manage care and transitions.
The Clinic RN participates in all performance improvement initiatives and must be knowledgeable of required clinical outcome metrics. Maintains active role in achieving patient volume benchmarks through support of physicians, nurse practitioners, and physician assistants.
This position is authorized to work in any location associated with HCHC within the approved scope of project by HRSA.
ESSENTIAL FUNCTIONS
- Demonstrates a thorough knowledge of preventative health measures, targeted chronic disease management and Medicare wellness exam processes.
- Provides knowledgeable reinforcement of provider's plan of care and supplemental care management services as appropriate based on the patient's needs.
- Performs and documents Annual Medicare Wellness services. Communicates identified needs designed to prevent adverse outcomes with provider and supports implementation of preventative measures as ordered.
- Provide immunizations and perform point-of-care lab testing (i.e. HbgA1c) as needed for gap closure.
- Works with patients/caregivers to develop active care plans to include goals. Provides follow-up with patient/caregiver to facilitate achievement of stated goals.
- Documents thorough medication review during patient encounters
- Collaborates with partnering organizations - including insurance companies, community resources, and other care providers to maintain continuity of care as needed or required.
- Promptly accept telephone calls from patients with varying levels of acuity and types of medical concerns.
- Promote clear and coherent verbal communication following an appropriate line of questioning.
- Assess clients based on the patient's own evaluation, concerns, and responses to questions.
- Evaluate, using approved evidence-based protocols, the significance of the patient's concern over the phone, determine if emergency care is needed, if a medical appointment should be made, if the concern should be reported to a provider, or if home-care advice is appropriate.
- Thoroughly complete documentation utilizing the appropriate computer software in compliance with the approved policies and procedures.
Requirements
- Registered Nurse License (RN) - Virginia
- Registered Nurse License (RN) - Compact/Multi-State License
- BLS required within 90 days of hire and maintained
- Minimum 1-year clinical experience as RN; ambulatory setting preferred
- Knowledge of the utilization of electronic medical records helpful
