Form of workAs a Baylor Nurse, you will work 4 days a week (Friday-Monday) providing Hospice Services and receive a full time salary and benefits. There is flexibility with the schedule but weekend coverage is required. * One year prior clinical experience as a licensed RN is required.
Advantage is currently seeking a Full-Time Registered Nurse to join our vastly growing Hospice team in the Pottsville, PA and Surrounding Areas! This position is responsible for providing patient care in their home.

- We provide on the job training including Client Assessment, Care Planning, Intervention and Overall Supervision of Patient Care in the home.
- RN Mentor assigned to provide one-on-one support
Responsibilities:
- Conducting Start of Care comprehensive assessments and completing OASIS timely
- Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete
- Preparing accurate, orderly, and timely documentation
- Coordinating care with patient and provide proper reports to Case Manager
- Provides health care instructions to the patient/family as appropriate per assessment and plan

Qualifications:
- An active RN license in the state in which practicing
- Minimum of 1 year of Clinical Experience as a licensed RN required
