Registered Nurse - Dcs In-Home Program (Sense)

We are actively looking for a PRN Registered Nurse (RN) to provide a variety of services to support DCS-referred SAGE clients, including conducting home visits, completing assessments with the families engaged in services, and being part of a collaborative team working with the family. SENSE (Substance Exposed Newborn Safe Environment) is a DCS-referred program which provides services for families after the birth of a substance exposed infant.

This is an in-person position, but allows for some remote duties.

This position has a flexible schedule, but includes occasional nights or weekends, as needed. Caseload size can vary depending on RN’s availability, and will typically average 9-15 hours per month.

SAGE has clients throughout the Phoenix Valley - we are looking for an RN in all areas!

Essential duties:

• Conduct in home visits within the first 30 days and last 30 days of services
• Assess the SEN (substance exposed newborn) child’s general health including physical condition, well-being, and postnatal mood disorders
• Assess child’s development and mother’s mental health status (Post-Partum)
• Provide infant safety resources including safe sleep practices, infant vision, feeding resources, developmental milestones, head shape, and other resources as needed
• Coordinate with Family Connections team members, attend coordination meetings, and maintain communication
• Attend monthly DCS SENSE Nurse calls
