ESSENTIAL DUTIES AND RESPONSIBILITIES
- Provides and collaborates with community partners to provide excellent community based medical care, education, and outreach, in accordance with FFH established procedures.
- Conducts role responsibilities at FFH sites and Community Medical sites based on scheduling needs, and as indicated by the program manager.
- Conduct COVID-19 testing and vaccine in the community, providing counseling in the event results are positive; complete contact tracing, managing isolation, and education to community members
- Performs testing procedures in line with current clinical standards, while adhering to hygiene, PPE, and infection control standards.
- Understands testing targets and goals for the test sites.
- Maintains proper medical supply inventory and advises program manager when supplies are needed.
- Follows all established PPE standards.
- In regard to vaccines:
- Demonstrates proper handling, storage and maintenance of vaccines.
- Documents vaccines accurately at all points of record keeping, including reconciliation in electronic record.
- Responsible for vaccine cold chain to include handling of vaccines.
- Accurately prepares and administers vaccinations with peer-checking technique as per standing and/or provider order and in compliance with all applicable federal, state, and local laws/regulations.
- Engages in positive manner when in the community, demonstrating professionalism and representing FFH's missions at all times.
- Assures that all documentation is correct and complete.
- Works with the Community Medical team, including medical provider, to provide comprehensive care community based settings.
- Prepares the environment to assure that the necessary equipment/information is available for effective and efficient care, both onsite at FFH and in community settings.
- Maintain medical records using standard filing systems.
- Performs inventory control and ordering supplies, as assigned.
- Initiates and performs rapid HIV test, including consent form.
- Receives cross training in other roles to support team members and medical operations, when deemed necessary by supervisor.
- Follows HIPAA procedures to maintain confidentiality and verifies the identity of individuals through "active" technique.
- Perform all other duties and tasks as assigned.
- Communicates effectively and respectfully with the healthcare team, peers, patients, community members, and families to ensure excellent service and best course of care.
- Completes comprehensive incident and patient complaint reports, following FFH policy, as needed.
- Collaborates with supervisor to identify areas needing improvement to include problem resolution.
KNOWLEDGE/SKILLS/ABILITIES
- Active Pennsylvania licensure for a Registered Nurse.
- Current CPR and Narcan Certification
- Knowledge of computer software applications (Microsoft Office Applications, others)
- Knowledge of grammar, spelling, and punctuation as it relates to correspondence necessary to perform essential job functions
- Knowledge of basic arithmetic to make calculations, balance and reconcile figures necessary to perform essential job functions
- Excellent customer service skills
- Skill in operating office equipment
- Ability to travel to Community Medical sites and be flexible with changes to locations/sites due to external factors (i.e. weather, cancellations, add-ons, etc.)
- Ability to adapt to working in outdoor elements, rain, snow, heat, in compliance with the departments Weather Policy
- Ability to work in, and respond to demanding environment.
- Ability to critically think which means the ability to think clearly and rationally about what to do or what to believe; understand logical connections between ideas; and identify, construct, and evaluate arguments
