Job description

Position Summary: This is a highly skilled position involving the medical treatment of individuals with Mental Health and/or substance use diagnoses.

Essential Functions:
1. Provide initial and on-going health assessments of CBMHP participants
2. Arrange for or provide follow-up medical care for CBMHP participants as needed
3. Ensure that there are written orders on file and updated for any prescription medications
4. Observe participant self-administration of medication in the office and the community, or administer if they are unable
5. Instruct participants about medications, side effects, precautions, and contraindications
6. Participate in formulation of medical treatment plans as appropriate
7. Notify case manager, supervisor and APNP/psychiatrists of any psychiatric symptoms or behavior changes that may require intervention
8. Serve as liaison with other collaborative medical professionals when necessary
9. Conduct blood pressures, weights, and any other medical treatments as needed
10. Document any participant or collateral contacts

* Other Duties and Responsibilities:*
1. Attend and participate in staffing and in-services
2. Participate in team meetings with case management staff
3. Conduct occasional home visits to give injections or evaluate medical issues
4. Provide updated license to the Nurse Supervisor
5. Other job-related duties as assigned

Job Qualifications:

Minimal Qualifications:

  • A two-, three-, or four-year degree from an accredited Nursing School
  • Licensure of a Registered Nurse
  • 1-2 years nursing experience
  • No convictions or license revocations that would fail a caregiver background check
  • Valid driver’s license, automobile and sufficient insurance to meet agency requirements.

Knowledge, Skills and Abilities: Good clinical skills especially related to medical issues, mental illnesses, and substance abuse disorders; knowledge of DSM V-TR, psychotropic medications and their side effects; ability to communicate effectively and in a positive manner with staff and participants; ability to respond appropriately in crisis situations; participants’ rights including principles of ethics and confidentiality; good organizational and computer and typing skills; effective use of time; ability to assist participants with medical and case management issues.

Work Relationship and Scope: Reports directly to the Nursing Supervisor. Provides direction to CBMHP staff regarding medical treatments and conditions. Has contact with other medical persons, clinics, and hospitals located in the community.

Working Conditions: Work is performed in the office, the community and in participants’ homes; participants may be unstable and symptomatic at times; much of the community outreach is done in the inner city; hours average at least 40 hours per week.

Physical Demands: Must be able to go up and down stairs and lift and carry 20 pounds to assist with participants’ medical or mobility issues.
