Full-time, 72 hours every two weeks, Monday-Friday, 8 a.m.-4:30p.m., every 8th weekend, rotating holidays and on call, working in the South St. Cloud Area.
Job Summary:
Join CentraCare’s “Team Teal” as a Case ManagerHome CareRegistered Nurse (RN), where you’ll serve people in your community and provide skilled care to patients in their homes. Home Care RNs work independently, allowing you to create schedules that work for you. You will manage a case load of 20-25 patients and care for around 5 to 6 patients per day. That allows you to give each patient the personalized attention and quality care they deserve in a less stressful environment. We provide extensive training and ongoing Registered Nurse office support while you’re making home visits to patients. This Case Manager Registered Nurse will serve the South St. Cloud area. You’ll earn an hourly wage plus mileage reimbursement while driving.
Superb Schedule:
- Monday-Friday 8 a.m.-4:30 p.m.
- 72 hours every two weeks.
- Every 8th weekend. With that, you’ll have a day off the week before and the week after your scheduled weekend.
- Rotating Holidays- 1 per year.
- Rotating on-call.
Duties Include:
- Providing skilled care in the home to prevent rehospitalization and improve compliance and accountability.
- Use of technology in the field including telemonitoring, computer and iPhone.
- Wound management and infusion therapy.
- Managing of high acuity patients via thorough assessments.
- Providing education and training to patients on their new diagnosis.
Pay:
You’ll make more but our minimum pay is: ADN $34.04/hr; BSN $34.89/hr; MSN $35.76/hr
Qualifications:
- Graduate from an accredited school of nursing required.
- Current Minnesota Registered Nurse license required.
- Current American Heart Association (AHA) BLS Healthcare Provider card required.
- Minimum of one year of RN experience in long-term care or an acute care setting preferred.
- Reliable transportation required.
- Valid Minnesota driver’s license, a driving record in good standing, and insurance liability coverage required.
$3,500 Sign-on Bonus available! To be eligible for the sign-on bonus you must be an external candidate who is hired into an eligible full-time or part time benefited position. Previous CentraCare employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire.
