Registered Nurse Case Manager

Responsibilities:
- Conduct patient assessments and develop individualized care plans
- Coordinate and collaborate with healthcare professionals to ensure continuity of care
- Monitor patient progress and adjust care plans as needed
- Provide education and support to patients and their families
- Advocate for patients' rights and ensure their needs are met
- Maintain accurate and up-to-date medical records
- Adhere to HIPAA regulations and maintain patient confidentiality
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Utilize case management principles to facilitate efficient and effective care delivery
Qualifications:
- Valid Registered Nurse (RN) license in the state of California
- Minimum of 2 years of experience in acute care or case management
- Strong knowledge of physiology, vital signs, and medical terminology
- Experience in ICU, ER, or behavioral health settings is a plus
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Proficient in using electronic medical record systems

Pay: $3,100.00 - $3,369.00 per week
