Job Description:
We are currently seeking a dedicated and experienced Case ManagerRegistered Nurse to join our healthcare team in Rochester, NY. As a Case ManagerRegistered Nurse, you will play a crucial role in providing comprehensive and coordinated care to patients, ensuring their physical, emotional, and social needs are met. You will work closely with patients, their families, and a multidisciplinary team to develop and implement individualized care plans to promote optimum patient outcomes.
Responsibilities:
1. Conduct thorough assessments of patients' medical, social, and emotional needs to develop a comprehensive care plan.
2. Collaborate closely with physicians, nurses, and other healthcare professionals to coordinate and manage patient care effectively.
3. Communicate with patients and their families to provide education, support, and ensure their understanding of the care plan.
4. Monitor and evaluate patients' progress toward achieving their healthcare goals and make necessary adjustments to the care plan.
5. Provide skilled nursing care, including administering medications, performing medical procedures, and monitoring vital signs.
6. Coordinate and facilitate medical appointments, diagnostic tests, and referrals to ensure continuity of care.
7. Advocate for patients' rights, ensuring their safety, confidentiality, and dignity are upheld throughout their healthcare journey.
8. Collaborate with community resources, home health agencies, and insurance providers to coordinate post-discharge care and facilitate transitions in care settings.
9. Maintain accurate and timely documentation of patient assessments, progress notes, and care plans.
10. Stay updated on current healthcare trends, evidence-based practices, and regulations to ensure the delivery of high-quality care.
Requirements:
1. Valid and active Registered Nurse (RN) license in the state of New York.
2. Minimum of 2 years of experience as a Registered Nurse, preferably in case management or a related field.
3. Strong clinical assessment and critical thinking skills.
4. Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals.
5. Ability to work independently and collaboratively within a multidisciplinary team.
6. Proficient in electronic medical record systems and other healthcare-related software.
7. Knowledge of healthcare laws, regulations, and reimbursement systems.
8. Ability to prioritize and manage multiple tasks efficiently in a fast-paced environment.
9. Flexibility to adapt to evolving patient needs and changing healthcare systems.
