Job description

Marin Community Clinics, founded in 1972, is today, a multi-clinic network with a wide array of integrated primary care, dental, behavioral, specialty and referral services. As a Federally Qualified Health Center (FQHC), we provide vital health services to almost 40,000 individuals annually in Marin County. The Clinics regularly receive national awards from the Health Resources and Services Administrations (HRSA). The Registered Nurse (Specialty Clinic Infectious Disease) sees patients both independently and as a vital team member in an integrated primary care model. Nurse works on case management of complex patients, health coaching, triage, and furthermore is able to improve patient care through participation in and development of quality improvement initiatives. Registered Nurse has direct patient-care experience in a community health or primary care setting. RN has desire to work with undeserved communities and a passion for health education and primary care. Registered Nurse assists with general medical care as well as medical case management for complex patients living with HIV and other communicable diseases.

  • Work schedule; Monday through Friday 8:00am to 5:00pm; no evening or weekend shift. 
  • $5,000 hiring bonus paid on first pay period. 
  • $1,000 hiring bonus paid after completing 90 days of employment

All employees are required to be fully vaccinated for COVID-19, including "health-care" required boosters as a condition of employment, subject to limited exemptions. New employees are required to provide proof of being fully vaccinated for COVID-19 and boosted before the first day of employment. If you completed your primary series early boosters,but have not received the most recent booster,you will be required to get the most recent booster and mask until completed if hired.


  • Provides culturally-sensitive, patient-centered care to all MCC patients.
  • Provides nursing care within scope of license and clinic policy and procedures.
  • Conducts assessments and triage of pediatric or adult medical complaints for walk-in or late patients.
  • Performs telephone triage of patients’ medical concerns and providing triage advice.
  • Reviews and follows up on Emergency Department and Hospital discharge summaries to facilitate coordination of transition to community.
  • Conducts patient co-visits with clinicians to provide additional support in chronic illness management.
  • Utilizes health coaching skills and encourages patient activation and self-management.
  • Participates in intensive outpatient case management of medically or psychosocially complex patients in the clinics.
  • Participates in Quality Improvement projects and PDSA cycles.
  • Measures PT/INRs and assists providers in dose titration and monitoring of anticoagulant medication.
  • Oversee STD clinic.
  • Manage patients who desire to start prep medications.
  • Fields Infectious Disease consult requests from providers and other members of the organization.
  • Coordinates care for external Infectious Disease referrals.
  • Provides case management services for HIV, TB, and Hep C patients.
  • Refers patients for insurance screening and assists with completing renewal paperwork for specialty insurance programs (Ryan White and ADAP).
  • Coordinates with external agencies in providing Ryan White services to HIV patients.
  • Coordinates with pharmacies to ensure specialty medications are received by patients in a timely manner to avoid gaps in treatment.
  • Performs continuous patient outreach to ensure Specialty Clinic patients remain engaged in care
  • Initiates referrals to public health programs and home-health programs.
  • Completes all required reporting to the Public Health Department for Infectious Disease patients and participates in case conferences as needed.
  • Manages in-box messages from patients and medical providers.
  • Administers injectable medications including insulin, antibiotics, and pain medication.
  • Administers routine immunizations per clinic standardized procedures.
  • Provides medication refills per clinic protocol.
  • Orders labs, medications, and vaccines per standardized procedures.
  • Reviews lab results, diagnostic imaging, and specialist consult summaries for actionable items.
  • Provides health education and formulating care plans for patients with chronic illness e.g., diabetes, asthma, heart failure, obesity.
  • Performs medication reconciliation as needed.
  • Performs nursing skills including insertion of straight or indwelling catheters, suture removal, wound care, and interpreting PPD skin tests, venipuncture, and sputum inductions.
  • Ensures appropriate maintenance of negative pressure room and orders supplies needed for sputum inductions.
  • Maintains and checks crash cart and AED.
  • Responds to medical emergencies in clinic and leads “MD STAT” situations alongside medical provider.
  • Provides support to Medical Assistants, including filling in as needed in the event of staff shortage or unanticipated need.
  • Coordinates pre-operative history and physical exams, including ordering labs or diagnostic studies per primary care provider.
  • Responds to in-person or electronic patient requests for lab results, medical questions and concerns regarding disease processes.
  • Other duties as assigned.

Education and Experience:

  • Associate or BS Nursing degree.
  • 1+ years of experience as an RN in any setting.
  • Active California RN License in good standing.
  • Current AHA CPR Certification.
  • Experience working with HIV patients desired.
  • ACLS or PALS certification.
  • Experience with case management of complex patients preferred.
  • Experience in a community clinic or primary care setting.
  • Familiarity or proficiency in using EPIC Electronic Health Records.
  • Experience with nursing care coordination/case management.

Required Skills and Abilities:

  • Excellent verbal and written skills.
  • Bilingual in Spanish skills are required.
  • Strong enthusiasm and willingness to pitch in whenever needed.
  • Must be able to work independently, with written protocols.
  • Must be sensitive to ethnic and cultural differences and their relevance to health care.

Physical Requirements and Working Conditions:

  • Prolonged periods of typing and sitting at a computer.
  • Must be able to lift up to 15 pounds at a time.
