The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco.  The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services. 

We advocate for health equity and deliver innovative, high-quality services responsive to the neighborhoods and diverse communities we serve with a focus on the Latino Spanish-Speaking Communities.

We are currently looking for qualified candidates to join our Shotwell Women's Clinic as a Lead Registered NursePlease note: this role requires an applicant to be bilingual (English/Spanish). This position reports to the Senior Nurse Manager and is a Regular, Full Time, Exempt role with a starting annual salary range of $83,000 to $93,000 with full benefits. 
Primary Objective:

The Lead Registered Nurse (Women’s Health) is a clinical leader responsible for the delivery of nursing care in Women’s Clinic (Gynecology & Prenatal) and provides clinical supervision of assigned Medical Assistants and other nurses (on-call, trainees/students) in the Women’s Clinic. The Lead RN provides comprehensive clinical support to the interdisciplinary Care Team within the Women’s Clinic and Prenatal Team and supports a panel of high risk, complex patient population. Lead RN supports clinical initiatives to support and maintain Patient-Centered Medical Home recognition. 
Essential Functions/Responsibilities:

Direct Patient Care (80%)
  1. Performs patient care activities within the scope of licensure. 

  2. Performs comprehensive triage and advice (phone, in-person) for women, adolescents, and pregnant/nursing mothers under the direction of licensed clinical staff.  

  3. Conduct comprehensive assessment of patients’ physical, behavioral health, and psychosocial needs. 

  4. Utilize behavioral strategies to help patients adopt healthy behaviors and improve self-care.  

  5. Processes medication refill and prior authorizations, in accordance with protocol. 

  6. Provides immediate response for triage (phone and in-person). Responds to codes within facility. 

  7. Performs nurse visits, under direction of licensed clinical staff, including but not limited to: 
    -Wound Care
    -Injections
    -Medication administration
    -Insulin and glucometer education
    -Depo-Provera
    -Blood pressure checks
    -Suture removal
    -Medication reconciliation

  8. Informs and educates patients on the significance of lab and radiology results, as directed. 

  9. Coordinate tracking of PPDs/QFTs, latent tuberculosis infection, and certain chronic conditions. 

  10. Complete Nurse Co-Visits in conjunction with Care Team in a timely, efficient manner. 

  11. Assist in the evaluation of women seeking birth control and conducts pregnancy options counseling, as needed. 

  12. Conduct eligibility/intake for pregnant woman entering the prenatal care program, as needed. 

     13. Facilitate care transitions for patients discharging from hospital and/or ED and receiving prenatal and perinatal care at ZSFGH in a timely manner. 
              -Ensure appropriate follow-up with patient’s Care Team. 
              -Review patient’s records, history, and reconcile medications upon transition into care. 
              -Act as a liaison to hospitals and other community agencies.  
              -Coordinating high-risk OB transfers to ZSFGH. 
      14. Oversees clinical chart preparation for assigned Departments. 
Clinical Leadership, Administration, and Other Duties (20%) 
       1. Supervises and conducts abnormal Pap and mammogram follow-up, and birth log follow-up, per protocol. 
       2. Clinically supervise assigned Medical Assistants and Registered Nurses. Coach nurses involved in providing women’s health-related services. Supervise MAs on tasks such as vital signs collection, phlebotomy, and wound care. 
       3. Work collaboratively with Clinic Manager to monitor, evaluate, and improve clinic workflows, including MA functions. 
       4. Actively participate in Comprehensive Perinatal Services Program (CPSP) activities, events, meetings. Oversee creation and maintenance of CPSP clinical/nursing workflows. 
       5. Attend and participate in huddles, trainings, and meetings, as requested by Supervisor.
       6. Participate in quality improvement projects. 
       7. Complete all required documentation in a timely manner such as medication orders and audits.
       8. Represent clinic and program to internal and external customers, and guests.
       9. Support maintenance and inventory of medical emergency equipment and sterilization supplies. 
      10. Maintains confidentiality of all patient information in accordance with federal guidelines and regulations. 
Qualifications:

  1. Valid, unrestricted Registered Nurse license in the State of California required. 
  2. Graduation from an accredited School of Nursing required. Bachelor’s or Master’s degree preferred. 
  3. Valid CPR and BLS certificate.
  4. Bilingual in English and Spanish required. 
  5. 6 months – 1 year of nursing experience required. Preferably in a primary care or community health setting. 
  6. Experience working in a collaborative, team-based environment required. 
  7. Experience in (or demonstrated interest) in women’s health strongly preferred. 
  8. Experience in ambulatory care and/or primary care clinic strongly preferred. 
  9. Leadership experience and desire to develop leadership skills strongly preferred.  
  10. Experience with electronic medical records and practice management (scheduling) systems required. 
  11. Familiarity with population health systems preferred.
