Job Description
A new home healthcare agency, servicing patients throughout Cook and DuPage County is on the hunt for a dynamic and compassionate Registered Nurse (RN) to join our team, primarily focusing on the Cook County Southwest Suburbs. The ideal candidate should be proactive, empathetic, and possess extensive knowledge in home health and geriatric care, coupled with previous home health experience. Role and Expectations:
As a key member of our nursing staff, you will play a crucial role in educating patients and their families on managing their health conditions, ensuring a smooth recovery process. Key duties include:
- Conducting initial and ongoing nursing assessments to craft and update personalized care plans.
- Collaborating closely with physicians and patients to tailor care plans based on comprehensive assessments at the start of care and during re-certification phases.
- Monitoring and evaluating patients’ health, providing direct care, educating patients and families on health conditions and treatment plans, and ensuring hands-on care when necessary.
- Ensuring integrated care by coordinating with other healthcare professionals involved in the patient’s care.
- Regularly updating the attending physician on the patient’s status and notifying them promptly of any significant changes.
- Engaging families in the care process to maximize patient recovery and resolution of issues.
- Supervising home health aides, ensuring adherence to the care plan, and conducting regular evaluations.
- Managing discharge planning and providing essential information for continued self-care post-service.
- Participating in Quality Improvement (QI) initiatives to enhance service delivery.
- Maintaining professional nursing standards as outlined in the Nursing Standards of Practice.
Qualifications and Requirements:
- Associate of Science degree in Nursing.
- Current RN license valid in the state of employment.
- Valid driver's license and willingness to travel.
- Ability to pass required background checks and drug screening.
- Strong written communication and documentation skills, with a preference for experience in electronic medical records.
