POSITION:
Nurse
RESPONSIBLE TO:
Nursing Director
SUPERVISED BY:
Nursing Director and Asst. to Nursing Director
POSITION/INDIVIDUALS SUPERVISES:
None
INTERRELATIONSHIPS:
Maintains open communication lines with facility staff, attending clinicians and physicians, patients and families. Liaisons with community agencies, vendors, and payor of health services.
POSITION PURPOSE
The Nurse is responsible for all Nursing related services for each patient and for linking patients to the physicians and to outside medical resources. They participate in the Interdisciplinary Team Meetings when necessary to inform the clinical team about medical issues and are available for emergency medical situations. The Nurse provides all services under the supervision of the Director of Nursing. The Nurse is responsible to provide medical & history screens, providing routine nursing care and overseeing the medical and psychiatric management of the patient in coordination with the attending physician. They are also a member of the Treatment Team and provide input into the development of the Master Treatment Plan.

1. ENVIRONMENTAL CONDITIONS:
Position is required to work in a temperature-controlled environment; 80% - 90% of the time spent indoors.
Potential exposure to violent situations.
Potential exposure to airborne/blood-borne pathogens or other potentially infectious materials.

JOB RESPONSIBILITY/FUNCTION:
SPECIFIC AREAS OF RESPONSIBILITY TO POSITION
  • Completes the following documentation within 24 hours of the patient’s admission to the program
  • Admission Note
  • Nursing Assessment
  • Pre-Screen Testing
  • HRS Consent Form
  • Patient Health Examination
  • Criteria for Admission Form (if Detox admit)
  • Transcribes Physician’s Orders and Implement all Medical Protocols as ordered
  • Administers medications and monitor detoxification process
  • Confers with and assist the Medical Doctor and the Psychiatrist.
  • Makes appropriate referrals using referral forms
  • Attends and participate in all 'in-service' functions
  • Assists with emergency situations and make immediate referrals
  • Provides documentation which reflects services for each patient in accordance with licensure standards
  • Maintains the Confidential nature of all patients and facility activities
  • Maintains required Training & Continuing Education
  • Maintains Standards of Ethical Conduct
  • Charge Nurse on each Shift supervises LPN’s
  • Complies with all company Policies and Procedures
  • Meets with patients requesting to see the doctor to determine the urgency of their medical needs and expedite treatment as required
  • Effectively communicates concerns about patient health to the Medical Director and follows up as necessary
  • Under the supervision of the Medical Director and Director of Nursing, assists in educating patients about health problems and medical care
  • Facilitates medication education for new patients and for all other patients starting new medications
  • Meets with therapists to provide information about patient medical problems
  • Assists the treatment team in incorporating medical issues into treatment planning
  • Takes verbal orders from the Medical Director, documents them in the patient record and carries them out accurately and professionally
  • Provides routine nursing care under the supervision of the Medical Director and Head Nurse
  • Participate in regular meetings with Clinical Director to discuss specific patient problems.
  • Facilitate Medical Groups as directed by Head Nurse
  • Participate in regular department meetings and staff meetings.
  • Encourage efficient utilization of supplies and equipment to prevent waste.
  • Maintains effective communication between outside medical services and the facility
  • Keeps and shares records and equipment
  • Participates in Performance Improvement, Utilization Review, Fire & Disaster Committees, and others as requested
  • Interacts with clients with compassion and respect
  • Attends in-service education programs as assigned to learn new forms, procedures, developmental skills, etc.
  • Has good work habits: get things done in a timely, efficient manner with no excessive absences; is on time for scheduled shifts and prepared to begin work
  • Functions well as a team member and contribute positively to the morale of staff
  • Performs additional duties, as deemed necessary by supervisor
  • Maintains up-to-date training in CPR, First Aid, Crisis Intervention, Infection Control, Fire Safety, and other areas mandated by organizational policies
  • Communicates effectively with other staff members and demonstrates the ability to work as a team member
  • Demonstrates flexibility and adaptability, responding positively to changing situations in the work setting
  • Demonstrates objectivity and maturity under stress

POSITION REQUIREMENTS/QUALIFICATIONS
1. EDUCATION:
A minimum of a Graduated from a Community College with a degree or diploma of Nursing LPN or Registered Nurse.
2. EXPERIENCE:
A minimum of 1 years experience in a chemical dependency/psychiatric setting, preferably in a detox unit and/or inpatient, partial hospitalization and/or outpatient setting required.
3. LICENSURE/CERTIFICATION:
Licensed to practice nursing in the appropriate state maintains current licensure per state requirements. Maintains current CPR.
4. SKILLS/ABILITIES:

Knowledge of Joint Commission Standards.
Demonstrates Proficiency in Communication & Written skills.
Knowledge of State & Federal Statutes Regarding Patient Confidentiality laws.
Knowledge of Drug-Free Workplace Policies.
Knowledge of Workplace Violence.
