We are seeking a Registered Nurse to join our team! You will be responsible for the assessments, care plan and discharge forms for clients.
Responsibilities:
- Complete new client assessments for home care.
- Establish patient care plans
- Report changes in patient symptoms or behavior
- Complete client discharge forms when clients are discharged from agency.
- Educate patients about health maintenance and disease prevention
- Complete 3 month sup visits for agency compliance
- Complete annual aid/caregiver competency checklist
Qualifications:
- Previous experience in nursing or other medical fields
- Ability to build professional rapport with agency and clients
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment
