JOB SUMMARYUnder general supervision, plans, develops, directs and evaluates patient diet and nutrition services; including warm handoff, individual consultations and classes in diet/nutrition. Collaborative with clinical teams and behavior health to develop individualized care plan, behavior goals and learning objectives with each patient. Provide individualized case management to patients. This position is intended to support the Maternity center patients and SCHC adult diabetic patient populations.
- Provide diet and nutrition counseling to a diverse population which may include, but is not limited to EIS patients, pediatric patients and their families, mental health patients, pregnant patients and patients with chronic disease, such as diabetes.
- Develop and record an individualized nutrition care plan.
- Document communications and Care Plans into electronic medical records (EHR).
- Conduct or develop diet/nutrition health education classes for patients, clinical staff and the community as needed.
- Conduct data collection and pre and post assessments as needed.
- Follow a process that involves a diabetic health and education assessment, development, implementation, and evaluation of an education plan, while taking into account psycho-social, economic, and language barriers.
- Assist in development of clinical protocols and educational content for diabetes self-management education programs.
- Collaboratively develop an individualized care plan, behavior goals and learning objectives with each patient. Provide individual case management to patients.
- Utilize a team approach to provide services and collaborates and communicate with team members when needed. Other duties and projects as assigned.
- Work closely with the Director of Quality Improvement to develop interventions specifically targeted towards patients with diabetes.
- Develop, implement and maintain ongoing Diabetes care activities in order to meet goals and objectives set with QI and Diabetes team.
- Assist team in the design of a minimum of one annual QI project, based on Diabetes. Collect Data, document activities and develop intervention plan.
- Attend ongoing Diabetes Team meetings.
- Work with QI, clinic management and staff to improve diabetes care measure for the clinic. Maintain knowledge of the latest techniques, equipment and products available for diabetes treatment, care and education
- Assist Manager in overseeing the services of each site.
- Willingness to work at multiple sites as needed.
- May include education and support on breastfeeding.
- Comply with established SCHC policies.
- Maintain confidentiality and security at all times.
- Support the mission of SCHC and its program.
- Other duties as required.
- Good communicator (orally, and in writing) with professional attitude in order to promote good relations with staff, patients and visitors.
- Ability to work well independently and as a team
- Precise and conscientious about details.
- Knowledge of Windows and word processing applications.
- Bachelor’s Degree in Nutrition or related field.
- Registered Dietician Certification.
- Experience in Microsoft Excel.
- Type 45 WPM.
- Excellent people skills and customer service orientation.
- Certified Diabetes Educator, Master’s degree in Nutrition or related field.
- Bilingual ability (English/Spanish) desirable.
