Schedule: M-F 8:00 am – 4:30pm
We are excited to share the salary range for this position is $72,000 - $81,000 annually, exclusive of fringe benefits. If you are hired at MOWOC, your final compensation will be determined based on factors such as geographic location, skills, education, and/or experience in addition to internal pay. Hiring at the maximum of the range would not be typical in order to allow for future & continued salary growth.
JOB SUMMARY: The Registered Dietitian (RD) is responsible for providing comprehensive nutrition consultation and education within the Medically Tailored Meal Program. This role involves assessing clients and their families for nutrition-related risks, providing tailored interventions, and collaborating with healthcare providers to address various nutrition needs, chronic disease management, nutrition support, regulatory compliance, and wellness promotion. Primary responsibilities include applying the Nutrition Care Process (NCP), including completion of the nutrition assessment, determination of the nutrition diagnosis, developing and implementing evidence-based nutrition intervention(s), and monitoring and evaluating the Client's progress.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Responsible for monitoring program performance, ensuring goals and targets manage referrals.
- Actively promotes the program's benefits and targets potential funding sources to support the program.
- Assumes responsibility for all case records, monthly statistics, and fiscal records.
- Conduct a nutrition assessment when warranted by a client’s needs or conditions
- Select assessment tools and procedures (matching the assessment method to the situation) and apply assessment tools in valid and reliable ways.
- Review dietary intake for factors that affect health conditions and nutritional risk.
- Evaluate the client and family’s knowledge, learning readiness, and potential for changing behaviors.
- Formulates and determines a 12-24-week meal plan focused on the problem's etiology and known to be effective based on best current knowledge of evidence from Evidence-Based Nutrition Practice Guidelines.
- Assesses educational needs and provides nutrition counseling based on individual needs, including nutrient requirements, knowledge and abilities, and medical, economic, and social situations. Identifies services that can be completed by the dietetic technician if applicable to the area of practice for the RD.
- Coordinate with those individuals and/or entities to ensure a seamless experience for the member and non-duplication of services.
- Maintains required records and reports in compliance with department, agency, local, state, and federal requirements.
- Attends and participates in all mandatory training sessions and meetings (including CPR and First Aid training) as state regulations prescribe.
- Demonstrates accountability for proper use of the clients protected health information imposed by regulatory requirements of the health Insurance Portability and Accountability Act (HIPPA) of 1996.
- Other duties and special projects as assigned.
EDUCATION, EXPERIENCE AND QUALIFICATIONS:
- Bachelor’s or master's degree in Nutrition, Dietetics, or a related field from an accredited institution
- Minimum of two years of experience working as a Dietitian preferably working with older adult populations
- Passed national exam and maintained ongoing education
- Registered with the Commission of Dietetics Registration
- Comprehensive knowledge and application of medical nutrition therapy and techniques for effecting behavior change.
- Critical thinking to integrate facts, informed opinions, active listening, and observations.
- Excellent counseling skills, ability to relate to a multi-ethnic community, and varied learning levels.
- Strong interpersonal skills to establish productive working relationships with multidisciplinary teams and support services.
- Ability to communicate effectively in written and verbal form to clients, the public, and medical staff.
- Demonstrates a “can-do” spirit, a sense of optimism, and commitment.
- Working knowledge of programs and services available in Orange County for seniors.
- Proficient in Microsoft Office Suite (Word, Excel, Outlook).
- Must pass a background check.
PHYSICAL DEMANDS & WORK ENVIRONMENT:
- This job operates in a fast-paced, multi-tasking, professional environment, requiring ongoing learning and complex problem solving.
- Ability to sit in a stationary position for extended periods and use both hands and fingers for typing on a keyboard.
- Ability to use office equipment such as a copy machine, printer, filing cabinets, and telephones.
- Occasionally must be able to move and lift office equipment and materials to 10 pounds.
- Close vision requirements due to frequent computer use.
- The noise level in the work environment is usually moderate (i.e., telephones, computers, printers, etc.).
- Occasional overtime and local travel may be required.
