Job Description
Responsibilities:
- Explain RMA program guidelines for client eligibility and health benefits coverage.
- Collect appropriate eligible documentation and other information to determine client eligibility for RMA.
- Process all RMA applications in electronic Refugee Eligibility Determination (eRED) and notice of change on behalf of the client.
- Assess and prepare client care plans based on the needs of the clients and ensure that their physical /mental health and wellness are met.
- Collaborate with other departments and program Case Managers such as Resettlement & Placement, MG, RCA, SAS, PC, and/or HP to ensure that the clients needs are met.
- Follow-up with all active RMA clients as necessary and ensure clients have their updated coverage information.
- Follow-up with USCRI as needed on RMA challenges.
- Responsible for accurately documenting case notes for all RMA enrolled clients.
- All documents must be completed properly before closing clients file.
- Maintain database of RMA clients and update their status on a regular basis.
- Complete the timely completion of all reports required by Program Manager.
- Attend bi-weekly or as-needed RMA programs meeting and monthly departments meeting.
- Communicate all activities, program success and challenges with Program Manager.
- Before making any determination, consult with Program Manager.
- Other duties as assigned.
Qualifications:
Some college degree in social work or related field.
Experience working for a social services agency, NGO, or other human services institution.
Client case management & and care plan assessments experience.
Community orientated and team player.
Fluency in English, with excellent written and verbal communication skills.
Foreign language fluency (speaking, reading, and writing), preferably in Spanish.
High degree of independent judgment, flexibility and problem-solving skills.
Valid drivers license, automobile liability insurance and knowledge of the city.
